  3. How to Apply for Your License Print out an Initial License Application from this website: http://bmt.ky.gov You will find it under “Resources” at the top of the page. Follow the instructions carefully. Complete the application and gather all of the required supporting documents.

  4. Be a copycat Make copies of your competed application and accompanying documents, including your check.

  5. about Transcripts • The KBLMT requires an official transcript from your school. • In order to be official, it must either be: • Mailed directly to the KBLMT by the school • or • In a sealed envelope that is enclosed with your application form. • A broken seal on the envelope will make it “unofficial” and it will not be accepted by the KBLMT

  6. Send your application Certified Mail with Receipt Requested so you know it was delivered and to whom. (You can only do this at the post office.)

  16. Allow 3-5 days for your application to be delivered. Remember, even one day late can mean a whole month’s delay.

  17. More Important information The Fiscal Section receives and processes your fee before your application is sent to the KBLMT Board Administrator processes your application. IMPORTANT: Check cashing or processing does not mean that your application has been approved!

  18. Our Board Administrator prepares all new and renewal applications. In January she processed nearly 150! Your application should be received 10 days before the Applications Committee review.

  19. The Applications Committee meets only once each month, 5 days before the KBLMT meeting. • If your application missed the deadline, it will be it will be held until the next month.

  20. The Board votes on the Applications Committee recommendations. The KBLMT meets only once each month. If your application isn’t approved, you must wait another month!

  21. It takes time to carefully post all licenses on the website after the meeting. • Allow 5-7 days to see your name on the KBLMT website.

  23. Reasons For Returns • Application is incompletely filled out • Application is unsigned • Correct fee is not included • All supporting documents are not included

  24. Reasons for delays • School Transcripts have not been received • School Transcript does not show conversion of credits to clock or contact hours as required by KRS 309.363 (1) (b) • Test scores have not been received • Disposition of felony and misdemeanors are not documented • Application is received after deadline

  25. Details, details! • Print or type your application clearly and neatly. • Complete all requested information. • Double check to be sure that your transcript has been sent. • Take and pass your exam well in advance of your application for licensure. • Don’t forget to include your fee.

  26. Special cases • If you check “yes” to felonies or misdemeanors, you MUST include documentation on the outcome of your case. No exceptions. • Persons with convictions will be asked to appear before the Applications Committee.

  27. READ AND KNOW THE LAW You are responsible for following our Massage Therapy Statutes and Regulations, found on the website. READ THE STATUTES AND THE REGULATIONS COMPLETELY.

  28. “Good to know” info Kentucky Board of Licensure for Massage Therapy (KBLMT) A seven-person board, appointed by the governor, that administers the laws that govern massage therapy in Kentucky.

  29. Communication The KBLMT Board Administrator oversees Three Boards and more than 4277 licensees. Many questions can be answered by exploring the website. Try there first. Email is the best form of communication; phone calls are the worst. Use mail for all official communications and keep copies.

  30. “Good to know” info KBLMT meetings are open to the public and you are welcome to attend. Check the website for day, time and location. http://bmt.ky.gov/ To address the Board and be placed on the agenda, you must make a written request to the board administrator at least 10 days before the meeting. You must include topics to be discussed so the Board can be prepared to discuss them with you.
