International Programs Office revised: 8 OCT 2009 Subj: Official Passport Application – Department of State DS-11 form • This form is used when the Traveler has a certified birth certificate to submit with the DS-11 application. A certified birth certificate has a registrar's raised, embossed, impressed or multicolored seal, registrar’s signature, and the date the certificate was filed with the registrar's office, which must be within 1 year of your birth. Please note, some short (abstract) versions of birth certificates may not be acceptable for passport purposes. Use this form if the traveler’s tourist passport was issued when the traveler was 16 years or less in age. Use this form if you have a Tourist passport but you need to keep it for other travel. • Access the US Department of State Travel website http://travel.state.gov • Click on Passports
4. On the left side click on Applications and Forms then select Application for Passport: DS-11 5. Click that you have read the Privacy and Computer Fraud notices. Then click Submit.
7. Enter in information about you. DATES are in MM/DD/YYYY format. Click Next. Enter Military Member or DOD Civilian Enter USN
8. The passport is to be mailed to International Programs Office (IPO) at this address: 117 Decatur Rd, Stop 13F Annapolis, MD 21402 In Care Of: International Programs Enter IPO’s mailing address here Enter No Midshipmen – enter your Home Of Record Address here.
9. Enter email address and telephone number. Click Next. Enter your _____ @usna.edu email address Midshipmen – Enter a cell phone number. Click Add Another Number button if you want to add additional telephone numbers (OPTIONAL) 10. Enter travel plan information. Date format is MM/DD/YYYY Click Next. For all trips, enter 5/9/2015, 3 Weeks, China. This doesn’t mean you’re going to China, we just need to enter information in these blocks.
11. Enter your emergency contact information. This should be someone who is not traveling with you. 12. Your most recent passport information. OPTION 1 - If you do not have a tourist passport book or card, select NONE.
12. Your most recent passport information. OPTION 2 – You have been issued a tourist passport book. Select Other Enter Submitting Birth Certificate You are to enter information about your tourist passport even though you will not be turning it in with your application for an official passport.
13. Enter information about your parents and spouse. Red Asterisks indicate required fields. Click NEXT. 14. If you are known by other names, enter the information here. Click NEXT.
Review all your information for accuracy and completeness. Make corrections using the Edit buttons as needed. Scroll to bottom of entries and click NEXT. 17. Once your review is complete, scroll to the bottom and click Next button.
16. You are applying for a “no fee” passport, also known as an “official” passport. You will not have to pay any fees. Click SUBMIT.
17. Information is presented about the next steps. Check this box Click Create Form
18. Click Open or Save. Either way you will get a 6 page document. ONLYpages 5 and 6 of the application need to be printed out. Turn in to International Programs Office.
19. A successfully completed DS-11 application will have a bar code in the upper left corner, will show DS-11 in the lower corner of the pages and is light orange in color. You can print out on black and white printer.
