  2. Crossing the Quality Chasm: A New Health System for the 21st Century, the Institute of Medicine

  3. Goals • Pharmacists involved in every step of the medication management process. • Enhance patient safety through pharmacist, patient and healthcare professional interaction.

  4. Task Assignment • For each chief of pharmacy to identify an unmet need at their facility. • Obtain buy-in from hospital leadership and stakeholders. • Creation of the clinic or service. • Monitor for intended outcome.

  6. Progressing…. • Throughout the year, periodic conference calls and email communication with the group. • Tracked the center responses and stored information on Sharepoint. We used emails and telephone calls to encourage submissions. • We also enlisted the VISN Formulary Leaders and others for support.

  8. Number of Clinics Created The individual and VISN level projects accounted for more than 125 new clinical programs and services.

  11. Staffing Additions • Medical centers had varying levels of staff added which was based on the needs of the service and the support received from administration. • There were approximately 115 FTEE added to support this mission.

  12. Enhancement of Care The level of clinical knowledge and expertise provided by the pharmacist dramatically enhanced the ability to provide appropriate pharmaceutical care.

  13. Conclusion With the creation of more than 125 programs and the addition of over one hundred staff, we accomplished our mission to enhance patient safety through the expansion of clinical services.

  14. Plans for the Future • Share submitted information including scopes of practice, justification papers, and clinic procedures. • Partner with the National Center of Patient Safety/PBM to work on opportunities and challenges across the VA system. • Seek energetic volunteers !
