Level 2 Services Referrals from primary care Referrals from secondary care Referrals to the specialist nurse clinics can be made through the Choose & Book • Type 2 diabetes • patients with sub optimal glycaemic control, not responding to treatment at level 1 • patients who require insulin initiation (see separate guidance for eligibility) • patients whose clinical indicators appear to be outside normal parameters • Type 1 diabetes • patients who are not currently receiving care at level 3 whose care can no longer be achieved optimally at level 1 • Desmond Structured Education • Please refer all newly diagnosed patients with Type 2 diabetes for DESMOND (Diabetes Education for Self Management Ongoing and Newly Diagnosed) structured education. • In Somerset, courses are run at regular intervals in different venues around the county. The course lasts for one day and can be booked through Choose & Book.
