Prescription • Through Care First • Most national pharmacies • $0 deductible • $5 generic, $10 Formulary, $25 Non-formulary • Mail order: 90 day supply $10, $20, $50 • Traveling and need additional supplies, call Member Services 1-800-241-3371 at least 10 days in advance
