Questions to be asked: • Do you have a defined policy on the communication of critical, urgent and unexpected significant findings? • Is this a Trust policy which has been agreed with your referring clinicians? • Do you have an automated electronic alert system to all referring clinicians with the ability for the clinicians to acknowledge receipt of reports and that they have been read? New cancer type Critical finding Eg pneumoperitoneum, pneumothorax, severe cardiac failure, severe pneumonia, acute fracture( other than emergency department requests) leaking aortic aneurysm, pyonephrosis, liver abscess. Urgent/significant/unexpected – Eg. malignancy, pleural effusion , metastases, DVT, ascites, hydronephrosis, pelvic mass, pancreatic mass, testicular tumour, Missed fracture eg femoral neck The Clinical Radiology Audit Committee’s 2014 Audit Forum
