  1. Severity 4 & 5 observations Aug ’2010 East Region

  2. Total severity 4 and 5 observation for the month of Aug.’10:- 22 Nos.

  3. Severity 4: – In order to fix the roof monitor purlin a worker came on the inclined bracings. _CSP(East) Action Taken: Activity stopped and counseling done. .

  4. Severity 4 :- Handrail missing on the top platform of scaffold. _Siemens Kharagpue(East) Action Taken: Scaffolding dismantled and provided another roof access.

  5. Severity 4 :- Lifeline ropes were found sagging on crane beam level. _Siemens Kharagpue(East) Action Taken: Activity stopped and life line fixed proper manner.

  8. Severity 4:- Roofing work started with out Access to Roof(Labour accessing roof by climbing on girt.) (Bafna Nasik-Prathmesh.) (MH) _West Action Taken: Work was stopped and asked builder site In-charge to provide Scaffold immediately.

  9. Severity 5:-Purlin was being used for Sag Angle fixing instead of Sag Angle Platform. (Bafna Nasik -Prathmesh.) (MH) _West Action Taken:- Work Stopped . Asked builder to provide Sag Angle Platform immediately.

  10. Severity 5- Life line post distance not as per STD. (Indo Euro -Suryodaya.) (MH)_West Action Taken: -Instructed Supervisor to maintain safe distance (Corrected)

  11. Severity 5-Scaffold erected not as per STD. & Checklist not maintained (Outriggers, Fall arrester, Platform, Rungs not provided.) (Indo Euro -Suryodaya.) (M)_West Action Taken:- Asked to re-Erect it immediately (Closed)

  12. Severity 5- Non Standard Rope Ladder was being used (Indo Euro -Suryodaya.) _(MH)West Action Taken: Removed from site.

  13. Severity 4- Life Line Bracket removed before completion of Secondary work .(Jyoti Plastic-R.K. Builder)(GJ) _West Action Taken:- Work was stopped and asked site supervisor put loose life line. (Closed)

  14. Severity 5- Hydra w/o Safety Latch & limit switch .(Jyoti Plastic- R.K. Builder)(G) _West Action Taken:- Stop work immediately. Hydra removed from Site and warning letter issued

  15. Severity 5- Flash Back arrestors provided only on torch side & fire extinguisher not available (Jyoti Plastic- R.K.Builder.) (G)_West Action Taken: Work was stopped and asked builder site In-charge to provide Flash Back arrestor both side immediately. (Closed)

  16. Severity 5- Assembly work was being done manually instead of mechanical lifting. (Jyoti Plastic-R.K. Builder.) (GJ)_West. Action Taken:- Asked builder Staff to provide hydra for same immediately. (Closed)

  17. Severity 5- Man Basket found at site. (Jyoti Plastic –R.K. Builder) (GJ) _West Action Taken: Work Stopped . Asked Builder In-charge to remove from Site.(Closed)

  18. Severity 4- Life line post not used for erection “T” column.(Linit Tech.–R.K. Buikder.) (GJ)_West Action Taken: Training imparted on “Safe erection procedure” and asked builder to provide life line post ( Meeting conducted with Builder)

  19. Severity 4- Non Standard scaffolding erected for de sling purpose.(Linit Tech.–R.K. Buikder.) (GJ)–West Action Taken:- Asked builder site supervisor to mobilized 4 sets of scaffolding 8 meter height with vertical ladder with fall arrestor.

  20. Severity 5 –.Manila rope used instead of U clamp for tying guy rope.(Linit Tech.–R.K. Buikder.) (GJ)–West Action Taken:- Asked builder to mobilize “U” clamp. (Closed)

  23. Severity 4-: Improper jointing of electric cable – (Perlos site site)_TN Action Taken: - Builder changed

  24. Severity 4-: Roof purlins not fixed completely no fall protection – (Perlos site site)_TN Action Taken: Instructed supervisor to provide fall protection and fix the purlins as per procedure Builder Changed

  25. Severity 4-: Worker found working on the rafter without any fall protection. – (Perlos site site)_TN Action Taken: Work stopped and instructed supervisor to provide fall protection (Builder changed)

  26. Severity 4-: Workers found working on non standard scaffolding- (Sheenlac site) TN Action Taken: Stopped work and asked to provide proper scaffolding (Closed)

  27. Severity 4-Grinding Machine being used without guard . –( Perlos site) TN Action Taken: Stopped work and asked to provide guard for wheel and Builder Changed

  28. Severity 4-: Naked wires found directly inserted in to the distribution board. - (Sheenlac site) TN Action Taken: Stopped work and instructed supervisor to used plug top (Closed)

  29. Severity 4-: Workers found working in unsafe manner – (Nokia seimens)TN Action Taken: Stopped work and instructed supervisor top provide access

  30. Severity 4-: Oil leak observed for 30 ton crane – (Nokia siemens)TN Action Taken: Stopped work and instructed supervisor to rectify the same.

  33. Severity 4-: Damaged Web sling been used for shifting of material – Carlsberg (AP) Action Taken: Instructed supervisor to replace with new one (Closed)

  34. Severity 4-: Scaffolding material been shifted without tying guide ropes- Jindal saw(AP) Action Taken: Stopped work and asked to provide guy ropes. Training conducted for all workers.

  35. Severity 4-: Life line not provided continuously - Carlsberg (AP) Action Taken: Stopped work and asked to provide life line

  36. Severity 4-: Ladder found used as sag angle platform ( Repeated violation). Carlsberg (AP) Action Taken: Stopped work and instructed to use sag angle platform

  37. Severity 4-: Compressed gas cylinders' without flash back arrestor and not positioned properly, poor LPG cylinder condition – Carlsberg (AP) Action Taken: Stopped work and asked to provide flash back arrestors (Closed)

  38. Severity 4-: Scaffolding side supports not provided properly and not anchored properly to the structure. – Omshakthi (AP) Action Taken: Stopped work and instructed supervisor to anchor the structure and provide side supports properly (Closed)

  39. Severity 4-: Workers found standing on purlins and working.- Omshakthi (AP) Action Taken: Stopped work and asked supervisor to provide proper working platform

  40. Severity 4-: Scaffolding wheels not having locks. Om shakthi (AP) Action Taken: Instructed supervisor to fix lockable wheels to scaffolding (closed)
