Optionals • Separate scissors for suture cutting, debridement and dissection (suture scissors, iris scissors and metzenbaums respectIvel (suture scissors, iris scissors and metzenbaums respectively • Forceps for clamping (hemostats) • Scalpel • German middle quality or better, not disposable are recommended • For plastic cases use tungsten carbide tipped needle holders
How to use a needle holder • Load body of needle at tip of jaws, 1/3 of way from swage and at 90 degree angle • Grasp holder with whole hand, no fingers in rings • Enter skin at angle of 90 degrees or more away from wound • Roll the instrument to give a circular needle track, if you can achieve a wider knot at bottom, this everts the skin
Arm at end of driver, perpendicular and at back 1/3 of needle
How to use thumb forceps • Hold like chop sticks • grasp superficial fascia not skin • or use one arm like a skin hook
How to control scissors for suture cutting • Thumb and ring finger in rings, index at fulcrum guides tips • For cutting knots on absorbables, slide tips down suture tails to the knot, rotate the tips slightly to avoid cutting it
How to use scissors for undermining • curved preferable, tips up • dissect bluntly, in plane between superficial and deep fascia
No touch option to minimize risk of blood born disease • Combined needle holder and suture scissors (Olsen-Hegar) • Combined tissue grasping and knot tying forceps (Adsons with teeth and knot tying platform)
