What this is about • Each year we complete a Health Self Assessment. • This year the form is different and is a bit harder to fill in. • The SHA will tell the Department of Health how well health services in our area work together for people with a learning disability.
What is working well (green) • With have a PMLD Subgroup for people with profound and multiple disabilities. The group are looking at the best way for staff at different sites to communicate with people. • We set up an Autism Assessment and Diagnosis Service (AADS). • We work better with people who also have a mental health problem or dementia. • We check that people (who are able to) give their consent for treatment. • The Hospital have good plans in place for MK Clinical Commissioning Group to take over safeguarding work.
We update information about people who are “out of area” to make sure they are safe. • Other areas contact us to make sure the people from their areas are safe in Milton Keynes. • The Check it out! Subgroup work with the HAT to look at what people think of health services using computer trackers. • Valuing People Consultants, Autism Trainers and Experts by Experience can check health services and help with interviews. • All hospital staff have to have Safeguarding training. Lots of staff in other health and learning disability services have the same training. • Learning Disability Services in MK have been joint between health and social care for a long time. • Milton Keynes Community Health Services (MKCHS) Showed the Partnership Board a report on their work.
What is working OK (amber) • GPs have lists of people with learning disabilities but we still need more information about people. • Over 45% of people have had an annual health check. • We started to use computer trackers to find out what people think about health services. • We are offering awareness training to pharmacies. • The Health Action Team (HAT) work with the Safeguarding Committee. • The Hospital want a nurse to work with people with learning disabilities and are holding a MENCAP road show.
People can have their own Health Passport if they want. • The HAT help other staff to stop people getting ill unnecessarily and to communicate well with people. • HAT have some drop-in clinics. • HAT work with the ECCO team to look at the health needs of people in transition. We are working with Northants on the Living Locally Project for young people with complex needs. • We are working with prisons on a learning disability assessment tool. • We use the Equality Delivery System to check that people are treated fairly but we need more information about people.
If providers need to work better with the Mental Capacity Act we tell them about it. Providers get visits twice a year from the Safeguarding Lead to check this. • We are employing some Experts by Experience to check that people who are out of area are safe. • Health and Hospital staff are on the Health Subgroup and HAT go to the Safeguarding Committee. • MKCHS Equality and Human Rights Committee covers Mental Health and Learning Disability.
What needs more work (red) • We will start using System1. This will help us make better referral forms from GPs. • We want to employ a Health Check Reviewer. • We need more information about Cancer screening.
What next? • 7th August - take to Partnership Board. • We need to answer some questions about numbers at the front of the form. • 13th August - take to Community Health Services Management Team. • 14th August - take to Clinical Quality Review Group. • 31st August or 7th September hand the form in to the Strategic Health Authority.
