Community Health Needs Assessment WHAT IS IT? Purpose: Identify and describe factors that affect the health of a community, and identify available resources to address those factors. Process: Formed advisory committee Collected and reviewed data Selected 8 priority health issues Identified future partners/resources Timeline: Began January 2018 Finished September 2018 Top 8 Priorities: 1. Mental Health 2. Substance Abuse 3. Older Adults/Aging 4. Housing 5. Access to Health Care Services 6. Tobacco 7. Obesity 8. Maternal, Fetal, and Infant Health

Community Health Improvement Plan WHAT IS IT? Purpose: A vision for the community’s health that addresses long-term, systematic efforts to address public health problems within the community. This is the community’s plan not the community health boards plan for the community. Process: Prioritized issues with advisory committee Narrowed down to top 3 priority areas with advisory committee Conducted a root cause analysis of top 3 areas Currently developing a structure where the root causes are the final priorities. Next Steps: Finalize structure, formulate goals, strategies, and define roles with partners Implement, monitor, and revise with partners Timeline: Begin September 2018 End December 2019
