Scale-up/Spread Recommendation Rating Survey • Web-based; N=126, response rate 39% (49 completes) • Respondent sample drawn from invitation list for Scale-up and Spread Conference (July 2010, Washington D.C.) plus approx. 25 additional respondents identified via colleague referrals • Respondents self-identified as: *Respondents were allowed to choose more than one response
