Identify Leadership experiences that meet the NAHQ Fellowship standard
Describe the types of published works,
presentations and mentorship that can be
used to meet Fellowship criteria
Relate attainment of Fellowship to improved leadership skills in roles which fosters
improved patient care.
3. Fellowship NAHQ (FNAHQ) Developed in 1996 by the Healthcare Quality Foundation
Currently 49 NAHQ Fellows internationally
Currently 7500 CPHQs internationally
List is available on the NAHQ website
Recognizes NAHQ members who have made outstanding contributions to the field of healthcare quality.
Honorees use the designation “FNAHQ”
4. Why obtain the distinction? Demonstrates dedication to leadership excellence and commitment in the healthcare quality field
Acts as a blueprint for an ideal career in the healthcare quality profession.
Provides the platform to stretch oneself.
Establishes a further goal once CPHQ is attained
Benefits not only those honored but those who seek role models in the healthcare quality field
Positively reflects on the national, state, and local healthcare quality associations.

6. Process of applying Use the fellowship application as a tool to guide your career
Develop your Strategic Plan to obtain FNAHQ
Declare your intent to apply by deadline (usually the end of January each year)
Will be assigned to a Fellow who mentors through the application process.
Application and supplementary materials must be submitted electronically
Submit all supporting documentation with application by April deadline with $200 application fee.
7. New for 2010 Leadership is now a mandatory category—must pick two other categories as well
Mentorship is better defined by template
Healthcare Quality related field must be defined and demonstrate proof that it is quality related
8. Fellowship Criteria NAHQ Membership (3 continuous years)
CPHQ required (Copy of Document)
Evidence of 30 hours of continuing education with topic, date, speaker and contact hours
Academic achievements and/or credentials
Associate Degree or higher OR hold additional certification or licensure
Copies of degrees/documents must be provided and labeled
9. Fellowship Criteria Employment Background documenting l0 years of primary responsibility for healthcare quality
Two years in supervisory/management position which may be for staff or project supervision
Job descriptions must be provided and labeled
Curriculum Vitae should be current and submitted
10. Quality related Applicant must demonstrate responsibility for leadership in healthcare quality management position OR healthcare quality related position.
Example: management and leadership of quality, information management for quality activities, performance measurement and improvement activities, or patient safety activities for the purpose of quality management.
11. Fellowship Criteria References:
Letter from two professional contacts describing your leadership skills, professional growth, and why you are an appropriate candidate for Fellowship.
Signed and dated letter from each contact containing 500 words or less on the sender’s letterhead
12. Fellowship Criteria Applicants must submit supporting documentation in Leadership (mandatory) plus two of the following:
Published Works
Lectures/Presentations
Mentorship
13. Leadership Service Leadership appointments or positions in associations and/or organizations on a local, state or national level
Demonstrates voluntary leadership and contributions to the field of healthcare quality
Document at least five different terms within last ten years completing at least one in the last two years
14. Published Works One personally authored or edited book where you are listed as the lead author or editor.
Two book chapters wherein you have authored a significant portion of the chapters.
Two articles published in professional peer-reviewed journals or periodicals whose circulation exceeds 1,000 readers where the applicant is listed as the lead author.
Combination of two: 1 chapter and 1 article.
15. Published Works Must be related to field of healthcare quality
Must include evidence the work was peer reviewed
Disseminated within ten years prior to the date of application
Unpublished accepted publications will be accepted
16. Lectures/Presentations
Evidence you served as presenter in 10 educational programs in the field of healthcare quality within past 10 years
Must not endorse the employer’s product or service
Must not be part of required job responsibilities
17. Lectures/Presentations
Must not be part of paid consulting responsibilities
7 out of 10 presentations must have different content
Paper and poster presentations are acceptable
18. Presentation Documentation Conference brochures listing you as presenter, program evaluations, and letters of acknowledgement from the professional organization on official letterhead
Course objectives and outlines
Copy of presentation or materials documenting length of presentation
19. Mentorship Serve as a mentor to at least two people in the field of healthcare quality
Must be voluntary and not part of your job responsibilities.
Document measurable goals/objectives along with specific examples of accomplishments.
20. Mentorship Submit statement written by the mentored individual in 500 words or less on appropriate letterhead addressing professional growth and knowledge base relative to your mentorship skills

Changes in the application requirements occur from year to year.
23. Preparation Begin by gathering the needed documents
Scan those only available in hard copy
When requesting references, note that letters must be dated within one year of the application submission
24. Exclusionary Criteria
Current members of the NAHQ Board of Directors are excluded from applying for Fellowship in the NAHQ
