  1. SDRC Accommodate:How to submit an Enrollment Application SDRC Disability Resource Center

  2. The SDRC Enrollment Application can be found by logging in to MyUWF and searching SDRC. The link to the application is also located on the SDRC website.

  3. Enter your UWF student information in the SDRC Enrollment Application. *indicates required field

  4. If you are requesting more than one accommodation, click on the Request Additional Accommodation button.

  5. If you have supporting documentation of your disability, you can upload the document by clicking the +Add Item button. A window will open to allow you to select your document to upload Use the following format for document title: Last First disability document

  6. Enter the Verification code as seen in the image. An audio file is available if needed to enter the code. Click Submit.

  7. SDRC will contact you to schedule an appointment to review your request for services.
