Incident Details Date: 8th March 2012 Location: Heavy Lift Barge adjacent to Platform Jacket Details: During personnel transfer from platform jacket to barge, utilising a Billy Pugh X-800 series transfer net, a Senior Surveyor fell from the basket and landed on a platform walkway. The fall from approximately seven metres and subsequent landing resulted in the Surveyor suffering head injuries from which he later succumbed in hospital onshore. Fugro Group

  3. Incident Details Basket was lifted from West side of platform with Surveyor Note decked landing area and use of tag lines

  4. Basket being lifted from east Side of platform with Surveyor And Barge Equipment Manager There was no landing area on this side

  5. Incident Schematic Basket landed between B3 and B4 transition piece to pick up the Surveyor plus one barge personnel

  9. Incident Schematic Basket continues to be raised whilst swinging and spinning, at this point injured Surveyor loses his grip and falls from basket (approx 8m), landing on walkway between B4 and E4 legs Injured party falls onto walkway

  10. Investigation Initial investigation report from barge contractor went viral, report was inaccurate and incomplete. Confidential data on the deceased posted on Facebook by unknown barge personnel. Company Regional HSE Manager, based in Singapore went to incident site to conduct further investigation. Company report reviewed by MD and Divisional HSE Manager, further questions raised and put to barge contractor for clarification Responses from contractor included in report findings Company Alert Notification distributed to all Company HSE Managers Updated Alert prepared for circulation

  11. Lessons Learnt There are many personnel transfer methods used by offshore construction and oil companies, all of which pose some level of risk. The method of personnel transfer must be taken into account by Project Managers when conducting assessment of risk for the project. The transfer methodology must be identified and mitigating factors must be agreed with the client prior to operations commencing. Mitigating factors must include environmental limit criteria, provision of adequate landing areas and use of the most appropriate mode of transfer. The identified requirements must be adhered to during the operational phase.

Further Actions Meeting with involved barge contractor : • To review the type of basket currently in use and consider an alternative which can provide impact and fall protection • To indentify and provide a proper landing platform for personnel basket landing in the initial jacket positioning phase

  13. Further Actions - continued • To re-enforce their standard operating procedure with regards to personnel basket transfer and lifting operations • To re-enforce the Stop Work Policy. All personnel have the right to refuse to be transferred or conduct a transfer when the situation is unsafe to do so
