Home The 5 StandardsPlease bear these 5 standards in mind when completing the daily records • I am encouraged and supported to be meaningfully involved in all my daily tasks and activities • I am given the maximum choice to decide what activity I wanted to do today • I am supported to visit my places, in my community to do things I like or to see people I know. • I decide who supports me and how my support is managed • I choose what I eat and have the opportunity to be involved in its preparation
Home What worked / What didn’t workBe mindful of the 5 Standards • Write up what went well from the person’s point of view. • This could be about activities, the right kind of support, an improvement in health or someone just having a good morning or afternoon (state why). • Record information that will add to everyone’s learning about how to help the person have more good days • Write up what did not go well from the person’s point of view • Again, this could be about specific activities, about someone not receiving the right support to do something successfully, or that the person seemed down (try and state why) • Record information that will add to everyone’s learning about how to help the person have fewer bad days. Home My Support Plan - Update Steven has made a clear choice that he prefers the ramps on the route 47 buses. Sections to change: 1. Home My Support Plan - updates The learning from this entry could lead to many updates in his support plan. These learning points should also be reflected in his 4+1 , and his support plan should be updated to reflect this change. For example: Sections to change: My One Page Profile (it may be that he wants to add something positive someone said to him in the club, or add that it is important to him to meet others when he is out, or that his support workers needs to be able to dance or at least have go!) My Support when going clubbing may need amending, adding in the learning about going to Pizza Express or dancing in the club, or meeting Jose) We may also have to record that it is important to Steven to get out, meet people and dance, and important for Steven to have accessible facilities and/or a confident support worker. For example: Sections to change: My Support when going clubbing may need amending, adding in the learning about not leaving too early, in turn this would add an action to the action plan about changing the rota accordingly We may also have to record that it is important to Steven to stay out late from time to time, and important for him that he has a flexible rota for his staff The Action Plan could include the action to contact the club and request a grab rail in the toilets. Home 4+1 Questions - Guidance This tool helps: • As a useful way to review actions and plan further actions. • By helping people to focus on what they are learning from their efforts. After quickly sharing their learning, time is spent focusing on future efforts. The 4 questions are: • Since the last time we got together last month….. • What have you/we tried? • What have we learned? • What are you pleased about? • What are you concerned about? • This is a simple but powerful tool for those who provide supports. The questions are useful when meeting in order to gather the collective learning in a way that leads to answering a fifth question. • Based on what we know, how should we move forward (what next?)?
