Margareta Söderström, GP Lund, Sweden, Ass. prof. in Copenhagen, Denmark. Lena Åström, GP Landskrona, Sweden Karin Ekelund, GP Eslöv, Sweden Sleep habits and sleep problems in the post-modern family. A study of children attending child day care centers ”Sleep you little willow bud”
Research question • To describe sleep habits and sleep problems of children attending child day care centres (DCCs) in Sweden.
Main findings • Shorter slept time • Changed sleep habits • Few parents regard their children have sleep problem
How did we come to this conclusion? • We choose DCC in Lund and Eslöv- same size and composition of children • Questionnaire + (Diary 14+14days) • 132 (93%) of eligible participated • 129 (98%) answered the Questionnaire
SLEPT TIME hours
Possible consequences of a shortened slept time • Reduced cognitive performance • Behaviour changes • Distorted circadiell rhythm • Increased crave for sugar • Negative effect on carbohydrate metabolism, endocrine system, increased blodpressure • Hamper the immunesystem
Why shorter sleept time? • Children go to bed too late? • Children wake up too early? • Sleep habits have changed? • To be a child of post-modern parents?
Sleep habits studied • Parental presence • Co sleep • Night waking • Long sleep latency • Early awakening
CO SLEEP %
Long sleep latency • 16 % • Longest sleep latency time was 2,5 hours • Parental presence at sleep onset were correlated to night waking (p=0.002) and co-sleep (p<0,001)
Night happenings • Snoring (8) • Nightmares (5) • Bedwetting (2) • Various (night terror, probable fear, wakes up before parents´s bedtime and restless sleep)
How parents knew their child had or had not got sufficient sleep.
Sleep problem Smedje, 3 times/week
Why more of night waking and co-sleep? • Snug time? • Parental presence result in more of night waking and co sleep • Increased acceptance?
The biology and the ´life puzzle´ • Children have to adapt to two systems • Sleep – a “zeit-geber” to the circardiell rhythm • Growing on insulin or growing on growth hormones? • Sleep problems behind the increase in weight among children?
Our messages • Put the children in bed in time • Learn them to fall a sleep by themselfe • No feeding during the night.
Lång sömnlatens, Nattvaknande, eller Snarkning Minst 3 ggr/vecka SLEEP PROBLEM ACCORDING TO SMEDJE
Night happenings • Wakes up before parents´ bedtime 8/129 • Snoring 8 • Night mares 5 • Bedwetting 2
1. Föräldranärvaro 2. Co sleep 3. Nattliga uppvakn. 4. Insomningssvårigheter
Main finding • Children sleep shorter time today than 10 years ago in all ages • Sleep habits of today were regarded as sleep problematic 10 years ago • The older the child the more often an owl instead of a lark • Children in DCC have a tight schedule 5 days a week
Could children be more healthy than they already are? Outdoor environment, infections, slept time, steps, sunexpo. and cortisol levels
Greatest problems for children in Denmark • Barn ær mindre syge jft med 2005. • Infektionssygdommer ær hyppigest Insatser infør fremtiden: bl.a akutte sygdomme – gør något, ex. minska antalet infektioner kroniskt syge, født med misdannelse - hvordan har desse børn det idag? laveste social grupper- insats før desse børns sundhet Danske børns sundhed og sygelighed, 2009
Children in child day care(nursery, preschool, vuggesstuer-børnehaver) • Abscense due to illness • Carriage of potential patogenic bacteria (GAS, pneumococci, Hemofilus Influenzae, Moraxella catharralis • Sleep habits-sleeping time • Symptoms of infections • bloodpressure • Outdoor environment • Steps taken (proxy for physical activity)
Findings from earlier studies • Sleeping habits have changed • Sleep time in children have decreased
Slept time timmar Hours Age in years
Possible consequencies of decreased sleep time • Cognitiv abilities decreases • Changed behavior • Increased craven for sugar • Negative effect on carbohydrate metabolism and endocrine systems, bloodpressure, increased weight • Influence the immunesystem
