Log on to www.omni403b.com and click on Participants for resources about the 403(b) plan, including videos, FAQs, and a savings calculator. See an example of savings and growth, and use the calculator to view available plans. Enter employer plan info to review plan details and explore fund options. Enroll in funds and complete necessary forms online or in-person. Contact Kathy in Payroll for any questions.

  2. Click Participants

  3. To the left are resources about the 403(b) including videos, FAQ, and a calculator

  7. To view the available plans, enter Massachusetts and Medfield Public Schools in lower left

  8. Enter Employer Plan Info and click Show Details

  9. Review the Plan Details for Medfield Public Schools

  10. Scroll over Fund or Investment Type for details

  11. Fixed Annuity Explanation

  12. Details of AXA Fund

  13. Clicking on the Fund name will take you to Fund page where you can Enroll

  14. On the Right side of the Plan Detail Page are all the necessary forms

  15. The Salary Reduction Form is used to authorize payroll to make the deductions from your check

  16. You can print a pdf and bring it to Payroll or complete the form online

  17. Enter the Service provider and Amount per Pay in $ not %

  18. You can use the Transfer Form to bring Funds in from an existing 403(b)
