JSSI FRAMEWORK: JAA AIM FOR SAFETY Under JAA overall objectives: The JAA aim at continuous improvements of its effective safety system leading to further reductions of the annual number of accidents and the annual number of fatalities irrespective of the growth of the air traffic
JSSI FRAMEWORK: THE TWO APPROACHES • Two complementary approaches are being used to develop the JSSI focused agenda: • The Historic Approach (Broadly based on CAST approach) • The “predictive approach” (The Future Aviation Safety Team)
HISTORIC APPROACH • Controlled flight into terrain* • Approach and landing* • Loss of Control* • Design related • Weather* • Occupant safety and Survivability • Runways safety* *: means common with CAST
Commercial Aviation Safety Team. The Commercial Aviation Safety Team (CAST) chartered the Joint Safety Analysis Team (JSAT) to review and analyse data on loss of control accidents, then develop and recommend interventions that could reduce the rate of loss of control accidents and thereby support CAST´s goal to reduce the fatal accident rate by 80% by 2007.
Commercial Aviation Safety Team. CAST then chartered the LoC Joint safety Implementation Team (JSIT) to identify the interventions recommended by the JSAT that were likely to be feasible and which offered high safety benefits. The interventions that were determined to have high priority were then consolidated into three broad project areas.
Commercial Aviation Safety Team. The three project areas and the safety enhancements approved by CAST for implementation were: Aircraft Design Policies and Procedures Training
Commercial Aviation Safety Team. CAST has evaluated all Safety Enhancements ( evaluation if a proposed safety enhancement could avoid or mitigate accidents that occurred to operators in a given region or worldwide) against category events, and have identified areas of unmitigated risks (remaining risk) not covered by the Safety Enhancements.
JSSI Action Plan Team. The task of the Action Plans Team (APT) was to develop work programmes (action plans) that define how each of the intervention strategies determined by the JSSI Steering Group for CFIT/ALAR/LoC could best be implemented, and to allocate priorities. Note The APT was not tasked with implementing any of the intervention strategies: its responsibilities was confined to developing work programmes that other bodies must apply in order to deliver the desired results.
JSSI Action Plan Team. The Action Plans Team (APT) has developed the Loss of Control Action Plans based on CAST Safety Enhancements: SE 26 – 34, SE 36, SE 39 – 41, SE 86 and SE 119. .
JSSIAction Plan Team • The JSSI/APT has monitored the CAST JIMDAT and RR JSAT progress and implementation of Safety Enhancements worldwide. • Statistics on distribution of full Airplane Fatality Loss Equivalents by accident location (worldwide), operator domicile and proportion of turboprop/jet airplanes has been provided to JSSI by CAST/ICAO. • JIMDAT Score Ratings has also been provided.
The 5 highest Score Rankings (LoC) • 18,3 (Flight Envelope Protection in New Aircraft Design), (CAST Study) • 15,5 (Flight Envelope Protection in Existing and Future Derivative Airplane Design), (CAST Study) • 11,4 (Advance Manoeuvre Training), AP 3.01 • 8,8 (Icing – Detection, Annunciation and Mitigation) (EASA Research), and • 6,4 (Dissemination of Essential Safety Information and Procedures) AP 3.04.
State of Play: CFIT/ ALA/ LOC CFIT and APPROACH AND LANDING Action Plans have been adopted by JAAC at their 04-1 Meeting. New “envelope” Completion Dates has been adopted (To be applied also to LoC Action Plans)
State of Play: CFIT/ ALA/ LOC Loss of Control: • Advance Maneuver; Policies and Procedures - SOP; Operational procedures Action Plans to be agreed with Operations Division before adoption by the JAAC • Risk management; Human Factors and Automation Action Plans put on hold pending discussions with FAA on CPS and the future of ATA Training Committee
State of Play: CFIT/ ALA/ LOC Loss of Control: • Flight envelope protection; Auto-flight; Icing;Displays and Alerting systems; Vertical situationdisplays Action Plans have been adopted by the JAAC/EASA. • Auto-flight; Icing; Displays and Alerting systems rely on harmonization work: • JAA NPA on Icing is out • JAA NPA on auto-flight should be out soon
Review of Action Plans Loss of Control • APT has proposed 33 Actions for implementation. • OST is involved in 25 Actions. • 2 Actions have been completed. • 11 Action are ON PLAN. • 8 Actions are put on hold pending co-ordination with other involved stake holders. (FAA and ATA) • 4 Actions have been withdrawn or deleted
Review of Action Plans Loss of Control • The 11 Actions which are ON PLAN have been aggregated into 7 Activitiesrequiring OST agreement: (at present time) AP 3.01.02, AP 3.01.03-05, AP 3.03.01-02, AP 3.03.03-05, AP 3.05.1, AP 3.05.05 and OPST follow up on Interventions Implementation.
Review of LoC Action Plans Action Plan 3.01.02 • JAA should publish the FAA Airplane Upset Recovery Training Aid on JAA website. • NAAs should copy The FAA Airplane Upset Recovery Training Aid for hard copy distribution as required.
Review of LoC Action Plans Action Plan 3.01.03-05 • The JAA should request operators, through their NAAs, that they review and by amendment ensure that their flight crew training syllabuses are reflecting advance manoeuvre training as supplied by manufacturers.
Review of LoC Action Plans Action Plan 3.03.01-02 • As a practical implementation (of the above topic), OST should request the OPSG/HFSC to review FAA AC 120-71A, the JSAT LoC Report, ICAO DOC 8186-OPS/611, and report to OST on the basis of the review if changes/additions are required to JAR-OPS or other associated material to provide information on circumstances, where there is a risk that confusion or misunderstanding could lead to loss of control. • Based upon the review carried out, revise JAR-OPS as necessary and publish in hard copy and on the JAA website guidance and ‘best practice’ that may be the result of the review.
Review of LoC Action Plans Action Plan 3.03.03-05 • The JAA should request operators, through their NAAs, that they review and by amendment ensure that their procedures, flight crew training syllabuses and inspecting requirements reflects ‘best practice’ guidance on SOPs designed to minimise risk of loss of control.
Review of LoC Action Plans Action Plan 3.05.01 • Add a requirement into JAR-OPS and JAR-145 that operators and maintenance organisations describe their process to update manuals and ensure that these are available to the relevant operation and maintenance personnel. • Request NAAs to put a review of such processes in their regular audits of operators and maintenance organisations.
Review of LoC Action Plans Action Plan 3.05.05 • Amend the Standard List of Reportable Occurrences in ACJ 20.x8 to include equipment failures that affect yaw/roll control.
Review of LoC Action Plans Follow-up on Interventions Implementation OST should charge the Operations Standardisation Teams, as specified in appropriate JAR-FCL/OPS Regulations, with the responsibility to include in their inspection items a follow-up on implementation of requirements produced by NAAs.
Update: Operation Sectorial Team • The Operations Sectorial Team is invited to: • Provide comments to OST WP 01-04, • Agree to the Action Strategies as stated in the Loss of Control Action Plans 3.01, 3.03 and 3.05, • Note the need for OST to re-address Action Plans 3.02 and 3.04, for agreement to proposed recommendations and return to the JSSI Steering Group, when these LoC Action Plans have been completed by APT,
