  3. Basics 3 year revenue grants Small grants Up to £6k (max £2k pa) Main grants No maximum – but be realistic!

  4. Timetable • Open now • Closing date 15th September 2014 • Committee 19th November 2014 • Outcome known 1st December 2014 • Start 1st April 2015

  5. Aims • Targeted services aimed at keeping people outside of the health and social care system • Universal services which build the capacity and resilience within individuals and communities. • Ensuring carers have recognition and support • Thinking beyond services to focus on what people want from life

  6. Eligibility • a voluntary organisation, community group, social enterprise or an individual working in partnership with a group of local residents; • has a constitution, or other governing document • an effective equalities policy • effective safeguarding policies for vulnerable adults & children • a recent set of annual accounts • can show how the project reflects the concerns and priorities of members of the group/community • can show how the project will have an impact on the health & wellbeing of participants.

Delivering outcomes Innovative projects working with adults and older people who use health or social care services, are socially isolated, discriminated against, or living in disadvantaged communities within South Gloucestershire & Meet two or more criteria for a main grant Meet one or more criteria for a small grant

  8. Criteria • Strengthening community capacity, working with and involving people who use services to deliver support which meet their health & social care priorities. • Promote positive ageing, active, engaged and informed older people. Supporting older people to stay physically and mentally well. • Reduces loneliness and isolation through participation and engagement.

  9. Criteria • Support people affected by dementia and/or Stroke to live well, be informed and active within their communities • Supporting people with long term conditions and/or life limiting conditions to remain independent and in control of their lives • Identifying and supporting carers to balance their caring responsibilities with living their own life, including facilitating the further rollout of ‘Do you look after someone' • Identification and support for carers of all ages. • Supporting people with mental health difficulties and learning difficulties to remain in control and engaged.

  10. What we don’t expect to fund • Activity that would ordinarily be purchased to meet individual need either as a commissioned service or via Direct Payments • Activity unable to demonstrate the potential to have a positive impact on reducing demand for adult social care services, improving outcomes for people who use services / carers. • Ordinarily we would not anticipate funding activity regularly delivered outside of South Gloucestershire unless there is a clear justification for the use of a particular venue.
