  3. Tele-clinic service • Quality improvement workshop • WEAHSN, CLAHRC West, PPI, NBT • Process mapped patient pathway • Developed patient information, patient eligibility criteria, System of undertaking blood tests in primary care, process of booking and appointment letters. #BHPhitconf

  4. Challenges • Change in OP booking system ( Lorenzo) • Failure to engage CCGs • Ensuring blood tests are done in primary care and results accessible prior to tele-clinic #BHPhitconf

  5. Ongoing work • Verbal confirmation from NHS England to pilot for transplant recipients • Lorenzo clinic build to be completed • Developing a SOP for all 6 pathology labs • Hope to start QIP in July/August • South Glos contract negotiation for 2017-18 for CKD patients (Continue to engage other CCGs…….) #BHPhitconf

  9. Renal study Day • Collaboration with UWE • ‘No fee’ study day • For primary care registered nurses • Feb/May 2016 – 20 delegates each • Very positive feedback (98% would recommend this to others) • August/October 2016 – fully booked #BHPhitconf
