  11. Upcoming Events & Calendar Hold your cursor over any event to see a brief description and access more information or save it to your calendar. Click “download” to save the event to your calendar client (e.g. Outlook) Click “read more” to see the event details

  12. Announcements & Forums Announcements are created by Community Admins to highlight events or resources for the community Community Forums are an area for the Community to discuss topics with other members of the community

Community Resources & Links, Photo Gallery Community Resources and Links area is used to share specific resources either within the HCP or on other websites The Photo Gallery consists of both a rotating image area on the Community Home page and a complete gallery that is accessible by clicking the image rotator

HCP Home Page – Broadcast your Community Send a request to Webmaster to have an image for your community placed in the Home Page image rotator If you have documents or resources you’d like to make publicly available, send a request to Webmaster. Once added to the resource center, they will appear here until other newer items are added Consider announcing events for your community in the information area of the Home Page.

Overview – Outcomes Sharing Site Use the Files menu to download templates, upload templates, enter data on the web entry form, and see information about your reporting history Use the charts menu to create charts that display National, Regional, State, and organization level data on any Outcome Measure Use the help menu to read FAQs about using the site and to request help from the webmaster when you need it Use the Account menu to update your in your information. For Team logins, this is where you go to choose or change your measures Look here when you login for important information regarding the Outcomes Sharing site

  27. Assigning your Measures Select all available measures for the selected population Enable Measure Checkboxes for the selected Population Save your measure selections 27

  28. Submitting Data Using a Form Select your team, then select the Reporting topic. Click Get Form when ready Enter Data for each Measure, noting that some measures require two data elements, while others only one After clicking Save Data on the form, you will receive confirmation your data was saved 28

  29. Submitting Data Using a Template Select Upload Data File Select the correct Reporting Month for the file you are uploading Once ready, click Upload Files Select Choose File and then browser to the file on your system Select the row that contains the correct Team and collaborative

  30. Single Charts Selecting View Chart allows you to create a single chart with one or more series

  31. Multi Charts Selecting Multi Charts allows you to create multiple charts, based on your selected Chart Detail option, also with one or more series

Drill Down Your chart will always show the options you have selected at the top On any chart, you can start at a national level and then drill down to Regional, State and Organization level data just by selecting a different option in the Locale selection box and then clicking Create Chart again

  33. Chart Details Holding your cursor over any data point will show you the details of that item

  34. Chart Filters Chart selection options include: • Date filters • Collaborative filters • Geographic groups • Organizations • Sites • Providers • Measures • Type of charts (line/bar) • Values shown (measure/ numerator/denominator)

  35. The HCP Help Desk • The helpdesk is always available to help answer questions, provide direction, and take care of your community’s needs • There are many FAQs, documentation, and even How-to-Videos available • Easily accessible • Full ticketing system
