  4. DS-870 Be sure to sign or the DMV will use the only date that it has. This could make it seem that you are very late. Employment must be listed for at least three years

  5. DS-870.1 An extra check mark. Not serious, but why raise questions?

  6. Driver’s Name is Reversed Did the driver have any tickets? Must be answered. Addresses for driver and carrier are reversed The Supervisor entered the wrong date. Always be careful with dates on forms Must Check Yes or No DS-872

  7. DS-873 The regs say that the DS-873 and the DS-875 may not be done on the same day. Good catch.

  8. Each item must be marked separately. A line is not acceptable DS-874 Don’t forget to date the form.

  9. Vision must be 20/40 or better in each eye Peripheral vision must be at least 70 degrees in each eye Be careful of corrections, they must be initialed. DS-874

  11. The doctor marked restrictions, but cleared the driver with no restrictions. DS-874 Remember that you can’t blame the doctor for filing incorrect paperwork. It is the carrier’s responsibility.

  12. The examiner qualified this driver even though he marked two ten point items. What fails this test? DS-875 The regs say that the DS-875 and the DS-873 may not be done on the same day. Good catch.

  13. The examiner must initial. DS-875Q

  14. The driver answered the question incorrectly, but the examiner marked it as correct. And again, the examiner must initial each page that the driver answers questions on. DS-875Q
