3. 3 Hospital Process of Care Measures Measures how often hospitals provide recommended
care to get the best results for adult patients.
Reporting Criteria:
Voluntarily submitted by acute care and critical access hospitals
All payer types reported
Process of Care Measures:
Eight (8) measures related to heart attack care
Four (4) measures related to heart failure care
Six (7) measures related to pneumonia care
Five (5) measures related to surgical infection prevention

7. 7 Hospital Outcome Measures Predicts patient deaths for any cause within 30 days of
hospital admission for heart attack or heart failure,
whether the patients die while in the hospital or after
discharge.

Original (fee-for-service) Medicare payer
Outcome Measures:
One (1) measure related to 30-day heart attack mortality
One (1) measure related to 30-day heart failure mortality

Standardized survey instrument for measuring patients'
perspectives of hospital care.

Voluntarily submitted by acute care and critical access hospitals

How often did nurses communicate well with patients?
How often did patients receive help quickly from hospital staff?
How often did staff explain about medicines before giving them to patients?
How often was patients’ pain well controlled?
How often were patients room and bathrooms kept clean?

The data is submitted voluntarily from acute care and critical care access hospitals
Children hospitals do not report because measures are strictly for adult populations
Some measures are exclusive to Medicare population
CMS query tool could be cumbersome for consumers, for example CMS divides information into North Florida and South Florida
How to address CMS multi campus reporting of those hospitals sharing the same Medicare Provider Number ?

Difficulty in downloading data (including linking information with AHCA facilities)
