  8. General Information and Forms Read the entire Solicitation document. Questions are submitted via NTTA Marketplace by pressing the solicitation’s Q&A button. All addenda must be acknowledged at time of proposal due date. Submit all required forms signed in blue ink.

  9. Don’t Become Non-Responsive or Disqualified! 2 1 3 Late proposals will not be accepted. This is an active Solicitation. Communication related to this Solicitation should only be with Procurement Services and Business Diversity. Communication with anyone outside these departments is grounds for disqualification

  10. Disqualifications (RFQ) Qualification must be submitted with response. The submission of price within qualification is grounds for disqualification. The Business Diversity form(s) must be submitted with qualification response. Non-submittal of Business Diversity form(s) is grounds for disqualification.

  11. Insurance Requirements Please have your broker/insurance agent review the insurance requirements in this solicitation to make sure your company can meet or exceed the requirements. A certificate of insurance will be required and reviewed for compliance prior to contract execution. 1 2

  12. Insurance Requirements Questions related to insurance should be submitted in writing. The written questions will be sent to the Insurance Program Administrator for response. The questions and responses will be released via addendum. 3

  28. Scope of Work • Construction management staffing and materials testing services for the PGBT 4th lane widening. Consultant will work as extension of NTTA staff. • Major items of construction work include: • Constructing 2 additional lanes (1 each direction) in median of PGBT • 13” Concrete Pavement • Full Depth HMAC between Belt Line Rd and Sandy Lake Rd • Pavement profiling south of IH 635 to include foam injection and diamond grinding • HMAC shoulder mill and overlay south of IH 635 • Reconstruction of tolling zone through MLP for NB entrance from Luna Rd. • Limited ITS infrastructure relocation (fiber backbone & laterals, CCTV cameras) • Full roadway and ramp restriping

  29. Scope of Work • Personnel: • Resident Engineer – Eli Lerma, NTTA • Chief Inspector • Field Engineer • Records Keeper • On-call specialty inspection (ITS, electrical, SW3P) • On-call material testing services • Field Office • Office space to be provided at NTTA

Scope of Work • Key expectations – field staff • Quality inspection (inspectors should have training in their specialty) • Attention to detail is vital • If something doesn’t look right, don’t ignore it. CM is last step in the quality process • 80% of the day should be in the work area • Roadway – look for soft spots, wrong elevations, grades, cross slopes, inlet locations, sign locations. Don’t let the contractor build it wrong. • Ordering and maintaining schedule of testing required • Coordination with adjacent projects

Scope of Work • Miscellaneous Requirements (not in RFQ) • Proper tools in every inspectors vehicle (plans, specs, shop drawings, tape measure, level, wheel, marking paint, string line) • Basic inspection gear is not a reimbursable expense (safety vests, small tools, etc.) • Phones are no longer a reimbursable item – every one has one already • Any leased vehicles on project must be identified with firm name clearly displayed • Viewing of 90% plans – contact Ms. Phaedra Hernandez (phernandez@ntta.orgor 214-224-2331 to schedule
