  3. AIM • Reduce incidence of falls (fall rate) by 40% from baseline by March 2011 • Reduce injury from falls by 40% from baseline by March 2011 • Increase the number of Fall Risk Assessments done on admission to goal of 100%

Change Ideas • Weekly audits at first to check for Falls Risk Assessments done on admission, monthly audits last 10 admissions • Found they were not filled in properly • Education provided, posted results for staff • Realized even if risk assessment completed, interventions not being put in place, falling star identifier not being used • Interventions put on the back of risk assessment sheet • Intervention Posters made for unit • Staff cleared a shelf for falls prevention material!!

  8. Challenges • Too much happening at once • Increase scope of practice for LPNs • RNs looking at their roles • Models of Care education • New policies • Lots of changes……Falls Prevention Team disbanded
