  10. BCBS Health Insurance

  11. Enrolling in BCBS Health You can ONLY enroll in health insurance within first 30 days of employment…. or
  12. Enrolling in BCBS You can enroll within 30 days of a change in family status such as: Marriage Divorce (or legal separation) Birth/adoption Losing coverage under parent’s plan at age 26 Spouse changing jobs Spouse retiring, etc. Court order
  13. Two Eligibility Periods for BCBS Within 30 days of employment Within 30 days of a change in family status There is no Annual Open Enrollment for health insurance.
  14. One Year Only Employees – Please read! If you are hired as a ‘one year only’ (on a temporary contract) for 2011-2012, you are NOT considered “new” the following school year 2012-13….. (……which means you need to enroll now!)
  15. Eligibility To enroll in health/dental employees must be at least .4 FTE For disability, .5 FTE For life insurance, .375 FTE If an employee isn’t eligible because they are below the minimum FTE, the employee has 30 days to enroll if/when FTE increases and the employee becomes eligible.
  16. When does insurance start? October 1, 2010 – for all insurance plans – first premium deductions are October 31, 2011 September 1, 2010 health insurance coverage is available – must complete an Insurance Waiver Form in addition to enrolling online. An Insurance Waiver form must be received in Human Resources prior to September 1. Waiver forms are available from Jane or Kyla.
  17. For September 1 BCBS health coverage: Premium deduction will be taken from September 30, 2011 paycheck Whatever dental option you choose will also start September 1
  18. Starting Health Insurance Sept 1 You won’t receive a BCBS ID card until October 1! Without ID card, you’ll have to pay for RX -- go to BCBS website, bcbsne.com for Drug Claim Form. Doctors/hospitals may wait for your card to process a claim – talk with your doctor’s office.
  19. BCBS Health Insurance – Waiting Period There is a 12-month waiting period for pre-existing conditions for health insurance. This is waived with prior “creditable coverage”. If more than a 63-day lapse of coverage, BCBS will NOT cover anything pre-existing. Consider the 63-day lapse when deciding if September 1 or October 1 coverage.
  20. Waiting Period Continued If less than a 63-day lapse, BCBS will require a Certificate of Creditable Coverage (Cert) to cover pre-existing conditions. The Cert shows when the prior insurance started and when it ended. The Cert is usually mailed to you after your prior insurance ends. When you receive the Cert and after you receive your BCBS ID card, send the Cert to the address on your card.
  21. Pre-Taxed Premiums Premiums are NOT taxed (for all insurance premiums except disability) District contribution is ONLY if enrolled in LPS health insurance

  24. What does health insurance cost?? Total monthly premiums: $518.41 for single health insurance $940.17 for employee/child(ren) $1064.21 for employee/spouse $1421.33 for family health insurance LPS pays a contribution towards the health insurance….

  26. District Contribution There is a District Contribution for Health Insurance only – employees pay full premium for all other insurance plans.
  27. LPS Married Couple Stipend If you are married with children and your spouse is an LPS benefit-eligible employee, you will also receive an additional contribution towards the family health premium. Example: If married to another LPS teacher, you will pay $109.75/month for a family policy. To receive the extra stipend, you must register as a married couple through ICE. Call Kyla or Jane with questions.

  29. BCBS of Nebraska Dental – 2 options If enrolling in Blue Cross/Blue Shield Single or Family health, Option 2 dental is included for the employee only. Employees can also enroll in employee/ child(ren); employee/spouse; or family dental Employees can enroll in dental and not health Option 4 dental, which includes orthodontics, also available

  31. Dental Insurance Dental can be added during any annual enrollment (see next slide for Option 4 rule)
  32. Option 4 Rule If you do NOT select Option 4 as a new hire, and change to option 4 at a later date during an annual enrollment, you must have been enrolled in Option 2 dental for at least 12 months, otherwise you will be limited to Coverage A ONLY for the first 12 months.

  35. Other Insurance Plans

  36. Vision Service Plan $12.44 employee $18.04 employee plus one $32.35 family Includes eye exam each year Lenses each year if needed Frames every two years You can enroll any year – during annual enrollment Not eligible to start sooner than October 1!

Disability Insurance Pays 60% of gross salary after annual/accumulated sick leave is used Disability insurance is REQUIRED for all new certified employees Premiums are based on salary and leave – and will be shown when you go through the online enrollment New retired teachers receiving a state retirement benefit can opt out (E-mail Kyla if you are a retired teacher)
  38. Disability Information - Continued Disability is also used for Maternity Leave (after sick leave is used) -- Maximum paid benefit combining paid leave and disability is 6 weeks for maternity. If you are having a baby – call Kyla for a Maternity Conference!
  39. Disability Insurance For a detailed brochure, go to the LPS website, click on Human Resources and Benefits. Effective date is October 1 (September 1 effective date isn’t available).
  40. Life Insurance – 2 Types $10,000 Death Benefit is provided at no cost to certified employees. You will provide a beneficiary when you go through the online enrollment. You can change beneficiaries at any time by contacting Kyla or Jane in Employee Benefits. Also Term Life is Available at a cost (next slides)

Term Life $1.90 per $10,000 term life – Maximum of $300,000 New employees can enroll in up to $100,000 term life without medical underwriting Term life available for spouse and children Double indemnity clause for accidental death Keep beneficiaries current! Contact Kyla or Jane to go through the online enrollment system to change beneficiaries.

Flexible Benefit Plan Medical Spending Account Lets you pay yourself back with pretax dollars for medical expenses NOT covered by any medical and dental plans Such expenses may include insurance deductibles and co-payments, vision and hearing exams, eyeglasses, contact lenses, contact lens cleaning solutions, orthodontics, etc. Complete list on payflex.com “Use it or lose it” Cannot include premiums from other insurance plans
  43. Flexible Benefit Plan Dependent Care Account You can’t do this AND take the tax credit on your federal income taxes Talk with your tax advisor to see which is better for you
  44. Flexible Benefit Plan For new hires, the plan year is October 1, 2011 through August 31, 2012 which is an 11-month period.

  46. Retirement Plans

  47. Retirement - NPERS Nebraska Public Employees Retirement System – all school employees belong 8.88% of gross pay deducted from each paycheck District matches 101% Defined benefit plan (retirement benefit is based on a formula as defined in the law) Vested after 5 years of creditable service Must work 15 hrs/week to be eligible

NPERS - Continued Can retire with full retirement benefit with “rule of 85” – Age plus years equals 85 with minimum age of 55 - or at 60 with 5 yrs. 1000 hours/year counts as a year of creditable service; partial year credit if less than 1,000 hrs Web site www.npers.ne.gov – school plan booklet available on the website Update beneficiaries if your family status changes! Complete a beneficiary form from Human Resources or they are on the HR and npers websites.

NPERS – Vesting Credit If you were employed with another Nebraska governmental entity prior to being employed with LPS, you have 30 days from your date of employment with LPS to make application for vesting credit. Contact the School Retirement System, phone 471-2053 if you have questions. It is your responsibility to have the application for vesting form properly completed and filed. Vesting credit is not included in the calculation of your benefits, and it is not a buy back. Application for Vesting Credit forms are available from Human Resources, phone 402-436-1593 or 402-436-1578.
  50. NPERS – Employment in another School District If you are currently employed by another school district in Nebraska and you are also employed by LPS at the same time, it is imperative that you notify the Benefits Department in Human Resources. This may effect your retirement.

  58. $225 Teacher Enhancement Fund $225 for each full time teacher (prorated amount for part-time teachers and those not working the full year) Used for classroom supplies, equipment, etc. Turn in receipts to your administrator or building secretary.

  60. Option A Leave A- Leave 14 days annual leave. Includes: 2 emergency 2 special 10 sick Pays $16.50 per hour after 20 years of employment upon separation Can accumulate to number of days in contract year
  61. Leave Option B B- Leave (Most popular option) 11 days annual leave Can be used at your discretion Pays $16.50 per hour after 10 years of employment upon separation Unlimited accumulation If unused, annual changes to accumulated the next year, and accumulated can only be used for sick
  62. Leave Option Deadline If no selection is made by September 30, employees will have Option B. You do not need to complete a leave option card if choosing Option B. If you choose Option A, you must turn in a Leave Option card which are available from Kyla or Jane.

Certificated Handbook (Continued) You will receive an e-mail from HR asking for your electronic signature acknowledging that you understand that you are to read and be familiar with the handbook, and that you understand the handbook contains: A Drug-Free Workplace Notice Anti-Discrimination and Harassment Policies Specific Complaint and Grievance Procedures which should be used for responding to harassment or discrimination If you change employee groups, you are to read and be familiar with the handbook(s) relevant to your new employee group(s). The handbook(s) is available on the LPS website, and if you do not have electronic access to the LPS website, please contact Human Resources to request the handbook(s) relevant to your employee group(s).

  70. Required Paperwork – NPERS Beneficiary Must be notarized – there are notaries in Human Resources If you have a change in family status– form available on the NPERS website, npers.ne.gov. to change beneficiaries.
  71. Name/Address Change Be sure to get new social security card if you have a name change and bring new card to Human Resources We ALWAYS need your current address! Change your address in ICE
