  4. First Page of Agreement: The Agency’s Direct Purchase of Services program is designed to promote the developmental of a comprehensive and coordinated service delivery system to meet the needs of older individuals.

  5. Scope of Services: List Services to be Purchased Defines the services/Measurement Only the service(s) contracted will be honored. Payment, Services and Reimbursement Sources of Funding Amounts Cost ( total & Federal/State Rate)

  6. Terms of Agreement: Describes what “you” (vendor) will do; a. Regulations b. Targeting c. Reports – Accurate & Timely d. Program Income e. Letter of Commitments f. Communication g. SAMS h. Accepting Gifts

  7. AAA Agrees to: Review Data ( i.e SAMS, Assessment) Communication – Clients Technical Assistance Quality Assurance Provide Policies/Procedures Start up funds – if applicable Employ full time staff

  8. Assurances Audits – due dates/requirements Termination: a. 30 days notice – Failure to comply with ANY part of the agreement Breach of Agreement: Lack of Proof – Insurance(s) Failure to Safeguard Asset(s)

  9. TERMINATION: WHY? Failure to meet line-item approved budget OR AAA may: Install Program Monitor Appoint Temporary Manager Cancel and/or Terminate

  10. Subcontracting Confidentiality Conflict of Interest Records & Financial Audit

  11. Indemnification Internal Dispute Mediation Participant Grievance Personnel – Key Insurance(s)
