INCIDENT During RIH 3 ½ IF drill pipe, the IP was opening drill pipe protector at rig floor for removing the D/P drift from the pipe. The IP inserted his right hand middle finger through the protector key hole before completely opening the protector. The stuck drift inside the drill pipe, came all of a sudden down and trapped IP’s right hand middle finger in between the drift and the protector, causing injury to his right hand middle finger. ROOT CAUSES: The IP failed to follow the basic safety rules/ procedure followed and known to all rig floor crew members at the site. The IP failed to fully understand the hazards while dealing with this type of protector with key holes (3x3 cms) The hazards related to protector with key hole were not identified /discussed in TBT even though hazards related to wrong body positions while drifting were discussed. Lateral Learning The safe drifting procedure to be reviewed to incorporate all hazards related to drifting. Ensure learning values are explained to all crew members. Recommended Actions: Discuss this alert in toolbox talks and HSE meetings and Distribute to all contractors Post on HSE notice boards Include in site HSE induction The IP inserted his finger through the key hole ( 3x 3 cms) before completely opening the protector Key Learning: Never insert finger through the key hole of the protector.
