Out-of-Pocket Changes • Coinsurance out-of-pocket maximums for MaxPlan Elite, CoreMed Elite and RightStart Elite: • 50/50 and 60/40 coinsurance OOP increased $1,000 • 70/30 and 80/20 coinsurance OOP increased $2,000 • Non-network deductibles and coinsurance out-of-pocket maximums: Note: Only 3% of Assurant Health PPO policyholders use non-network providers

Additional Product Enhancements: The Assurant Health Elite Individual Health Insurance Portfolio is further enhanced by IM product changes. • Dental Plus—higher-level benefits for dental insurance, including $75/visit for semi-annual well services • Office visit copay available with $5,000 and $10,000 deductibles—on MaxPlan Elite and CoreMed Elite plans • Maternity benefit waiting period: 90 days for all plans • Maternity option – for RightStart Elite and SaveRight HSA Elite (in states where approved) • Deductible credit pilot: • New customers: 8 month pilot program that credits 10% of the selected deductible when proof of prior coverage is provided* • Inforce customers: First quarter test providing 10% deductible credit in 2008 for selected renewing customers * Available with MaxPlan Elite, CoreMed Elite and OneDeductible Elite.
