SeaMar Community Health Clinic • Originally began as a safe space for migrant workers and the Latinx community. • Seattle—Marysville • First opened in South Park Seattle. • Second opened in Marysville. • Now the largest comprehensive health and human services organization in Washington State. • Serving 11 counties.
Most Commonly Used Services Showers • 500 showers a month. • Monday—Friday • 8:00 AM—3:45 PM • 2 showers available. • Beginning of the month always the slowest. Laundry • 26 loads—Normally around 50 loads. • Monday—Thursday • 8:00 AM—2:00 PM. • 4 loads max per day. • Washer down since 4/16/19.
Our priority as Homeless Case Managers is to get patients scheduled with the doctor on site, or connect them to other necessary medical personnel. In hopes of reducing emergency room occurrences/costs through preventative healthcare measures.
Access to Medical / Dental / BH • Access to the all Sea Mar schedules. • Ability to input new patients in the system. • At 11th street Medical we have: • Medical Professionals (Doctor & Nurse Staff). • Substance Abuse Counselor. • Homeless Case Management. • Dental (across the street).
Access to Other Community Resources Short Term Goals • What can we do for them right now? • in the next few days? • Some clean clothes? • Doctor’s visit? Dentist? Long Term Goals • How can we assist them in the long run? • In the next few weeks? • Housing • Regimented medical care mgmt.
Clothing & Food Bank • Provide necessary clothing items. • People have to set up a meeting with Case Managers to get clothing. • Great way to get people set up with Case Mgmt. • Socks, jackets with hoods, etc. • Food Bank—To be determined.
As Case Managers we are… • Meeting people where they are at. • Removing barriers and finding open doors. • Helping them find motivation within themselves. • Constantly using words of affirmation. • We are there for the day they decide they want change.
