Why such a program? • Diabetes is common in HK • Multiple complications require multiple specialties • We need a network to link up the GP, specialists, allied health, tertiary hospitals and the industry
Aim of the program • Enhance the co-operation between the primary care and secondary care doctors • Enhance the co-operation between the private doctors and the tertiary care hospital(s) • Improve the standard of diabetic care • Reduce the incidence of complications
Target group • GP and specialists with interest in diabetic management • Clinic staff to help the doctors
Highlights of the program • A certificate course for GP in the area • A course for the clinic assistants of private clinics • Publish handbook (GP, specialist , allied services) • Internet discussion group • Resources Centre • Patient support group
Certificate Course for GP • Aim: update knowledge on DM • Improve treatment skills • Improve team approach • Certificate to indicate interest of the doctor in DM
Course for clinic assistants • Aim: train the clinic staff to help the doctor • Contents • Basic concepts on DM • BMI, waist measurement • BP and blood sugar monitor • Insulin injection and training\
Program Handbook • List of GP and specialists with interest in DM management in the area • List of allied services • Indications for referral to specialist and tertiary hospital(s) • Protocol for referral to HA hospitals • Advertisements
Internet discussion group • Discussion group on diabetic management • Diabetic news • Special offer from the industry

Reference Centre for urine microalbumin • One of the services to members of the network • Members can send urine for microalbumin test for diabetic nephropathy • Urine test for microalbumin with Microtek • If positive, confirm with DCA 2000+
Future direction • Agree on Treatment protocol • Encourage patients to attend doctors in the network group
Implementation Plan • Design the program • Identify partners – QEH, UCH, Diabetes HK, Nethersole community service • Recruit members -GP and specialists • Contact allied health and industry • Date of launching • Set up working groups
