AIM Statement: (What are we trying to accomplish?) in the SMART format (Specific, measureable, achievable, relevant and time-based) (QI) OR a Research Question in the PICO format (patient, intervention, comparison, outcome). Background: At least one paragraph describing the problem in a more global way (summarize the literature, problem, measures used in other studies and prior successful interventions). This should discuss the general nature of the problem/process. Why is this generally an important problem to address? (Think of the 6 AIMS for health care quality (Safe, Timely, Effective, Efficient, Patient-Centered, Equitable) What has been tried in other settings in the past to address it? Conclude with a discussion of the problem within our local setting and any prior projects related to it. Purpose: Why is your specific project being done? Methods: How is (was if completed) the project being done? What is the intervention, and when will (did) it occur? What things are being measured? (Think of Outcome, Process and Balancing measures. Include variables to be collected and your data analysis plan) Conclusions and Next Steps: Conclusions from the project’s results, preliminary results and areas for future study. • Results: • Present any pilot, chart review or other data obtained. What You Learned: What did you learn about the topic of your project? Did you experience interdisciplinary teamwork?
