  3. Copies will be needed of the following items: Birth Certificate Social Security Card Medicaid or Medicare Card Social Security Benefit information Current Doctors information Psychological Report Individual Education Plan (IEP)‏ Information for Applying

  4. A copy of the Psychological Report and Individual Education Plan (IEP) which can be obtained from your child's classroom teacher. Where do I find this information?

  5. Sign: Authorization for Release of Information Notice of Privacy Practices Sign Release Forms

  6. Place all the forms and copies in the self addressed envelope and mail to Augusta Complete the Application

  7. This completed packet must be sent to: Department of Behavioral Health and Developmental Disabilities Region Two Office Attn. Intake and Evaluation Unit 3405 Mike Padgett Highway, Bldg 3 Augusta, GA 30906 I have completed paperwork. What is next step in this process ?
