  7. Electronic Health Record In 2011: • Approximately 80% of health centers had EHR • Approximately 55% of all other ambulatory care providers nationwide had EHR

Uniform Data System • UDS tracks information from Health Centers • patient demographics • services provided • clinical indicators • utilization • Reported at the grantee, state, and national levels • Used to improve health center performance and operations, and to report overall program accomplishments

  9. Current Uses of UDS Data • Ways the UDS data are currently being used • UDS Mapper • Find a Health Center app • iTriage

  10. UDS Mapper HRSA funded- Bureau of Primary Health Care collaborated with John Snow, Inc. and the Robert Graham Center to develop a mapping and decision-support tool driven primarily from data within the UDS, previously not publicly accessible at the local level.

UDS Mapper • The information available in UDS Mapper • Data mapped by ZIP Code Tabulation Area (ZCTA) from US Census • Point location of health care providers • Medically underserved areas/ populations • Health Professional Shortage Areas (Primary Care) • Geographic boundaries and highways • Topographical maps and street maps • Uninsured • % without dental visit in past year • % without a usual source of medical care • % who have delayed or not sought care due to high cost

UDS Mapper • The UDS Mapper can be used to assist in evaluating the geographic reach, penetration, and growth of Health Center Program and its relationship to other federally-linked health resources • Audience of the UDS Mapper: • Policy makers and planners • Researchers/ Social Service/ NGOs/Faith Based entities • HRSA project officers • Primary Care Associations and State Primary Care Offices • Current and prospective grantees • Staff from other types of health centers

  14. Find a Health Center App • This app allows the public to easily find their nearby health center by typing in either an address or state/county

  15. iTriage • iTriage is not supported by HRSA, but it is a tool that uses publicly available UDS data to provide health center (HC) locations to patients • Provides relevant healthcare-related information • iTriage retrieves HC location data from the HRSA Data Warehouse • http://datawarehouse.hrsa.gov/webServices.aspx
