Who is Your Community Health Center?

  7. Financial hardships • Creates gas money concerns • Reliable transportation from family and friends • Distance between home and clinic • No pharmacy pick up included. Example: extra cost with taxi service due to additional stop Barriers of Transportation Reported by Patients and Observed By Office Staff

No routed public transportation system offered- no city bus routes • Logisticare for Medicaid patients • We team up with local transportation companies such as SMTS (Southeast Missouri Transportation Service), Licking Bridge Builders, and OATS Transit (Operating Above the Standard Transit) • Local taxi services are utilized Transportation Outlets in Phelps and Surrounding Counties for Patients

  9. We Identified a Need (Transportation) • Options: • To partner with community organizations • Buy vehicles • Contract with a ride share service • YCHC chose the third option. So YCHC’s Plan?

  10. Funding? • SUD and BH non-allocated funding sources • HealthTran So YCHC’s Plan?

  11. Program Launch in February 2019 • Services Phelps county and surrounding 5 counties • Average 40 scheduled rides per month • 1.73% drop in cancellations from last year • Perks: • Pharmacy stop available after appointment • Same day transportation available YCHC Health Tran Program

Better Tracking in EMR of Patient Cancellation/No Shows • Add Registry into DRVS for Tracking of Health Tran Patients • To help track quality measures on patients (ex: complaint with medications, attending follow up appointments with provider) • Marketing • Recruitment of Volunteer Drivers YCHC Homework
