Carolina Beverage PPO Plan The PPO Plan provides a full range of covered medical benefits. • With a PPO, you have a choice each time you need care. You must use providers in the PPO network or pay higher out-of-pocket costs. • You do not need to choose a Primary Care Physician.

Dental Schedule of Benefits Benefits are based on the 90th percentile of Reasonable and Customary
Basic Life and AD&D Insurance We all want the best for our loved ones, especially if the unexpected should happen. Life insurance provides protection for your family in the event that you are no longer able to provide for them. • Carolina Beverage Corporation provides eligible employees with basic life and AD&D (accident death and dismemberment) insurance in an amount equal to $15,000 as of your date of hire. • This benefit is provided at no cost to you. • As an eligible employee you are automatically enrolled, but you will be responsible for designating a beneficiary.

Optional Voluntary Term Life Insurance Carolina Beverage Corporation also provides the opportunity for you to purchase additional life insurance for yourself, spouse and dependents. All salary and hourly employees are eligible as a new hire or at each Open enrollment. Here’s how it works: • Employees can purchase up to $100,000 in coverage without answering medical questions. $20,000 can be purchased for spouses and $10,000 for dependent children with no medical information required. Please note that employees must purchase life coverage on themselves in order to purchase spousal or dependent coverage • If you do not enroll within 31 days of your first day of eligibility, you will be considered a “late entrant” which may require evidence of insurability • You must select your beneficiary when you complete your enrollment application
Guaranteed Issue Employee: Minimum-$10k Maximum-$300k $100k Guaranteed. Spouse: Minimum-$5K Maximum-100% of employee coverage up to $150k. $20k Guaranteed. Employee coverage is required to elect spousal coverage. Children: $2500 or $10k benefits available.
Voluntary Short-Term Disability • All Carolina Beverage Corporation eligible associates are offered the opportunity to purchase Voluntary Short-Term Disability coverage. • When eligible, you may purchase coverage in the amount of 60% of your monthly earnings, subject to a maximum benefit of $2500 per month. • Benefits begin for disabilities caused by accident or sickness on the 14th day of partial disability • As long as you remain disabled, Voluntary Short-Term Disability benefit payments will continue for up to 180 days. • Please note that if you enroll after your enrollment period, evidence of insurability will be required for all coverage amounts.
Voluntary Long-Term Disability • All Carolina Beverage Corporation eligible associates are offered the opportunity to purchase Voluntary Long-Term Disability coverage. • When eligible, you will receive a monthly benefit of up to 60% of your pre-disability earnings, subject to a monthly maximum of $5,000. No medical evidence is required if you enter the program as a new hire. Evidence of Insurability is required after initial offer is refused. • You must be disabled for at least 180 days before you can receive a Long-term Disability benefit payment. • Benefit Duration is to Age 65.

Start enrolling 30 days after hire date. Window will close 120 days after hire date.
