  1. State Health Plan members …some services require preauthorization BEFORE you receive them. Call Medi-Call at 800–925–9724 or 803–699–3337 to get things started if you or a covered family member need: • any type of inpatient care • septoplasty, hysterectomy, TMJ or jaw surgery • sclerotherapy or any potentially cosmetic surgery • an MRA, MRI, PET or CT Scan • chemotherapy or radiation (new course) • home health, hospice care or medical equipment • any fertility procedure • evaluation for a transplant • outpatient therapy after inpatient therapy. Also, you must notify Medi-Call of a pregnancy during the 1st trimester to avoid a financial penalty. Although your doctor may call Medi-Call, it is your responsibility to see that it is done. Remember, there are penalties for failure to call Medi-Call. Refer to your Insurance Benefits Guide, beginning on page 39, for more information.
