Global View of CMC Program-Services Services structured in 4 groups • Four dimensions of health: Physical, Spiritual, Emotional and Psychological • Cree life cycles: Children, Youth, Adults/Elders and Current Services
Global View of CMC Program-Services 4 groups of services • Current and ambulatory services: general health and social services • Awash services: program/services for mothers and children 0-9 yrs • Uschiniichisuu services: cover the common needs for youth 10-29 yrs • Chishaayiyuu services : take care of common needs for people 30 and over yrs
Main Target Clientele • Children and Youth aged 10 to 29 yrs old and their families, including children with special needs CBHSSJB, 2015
USCHINIICHISUU Services Main Goal • Help foster health and well-being in Eeyou youth by addressing their developmental needs so as to allow them to go through the transitional period towards adulthood. With a concentration/expertise that: • Provides care and support for individuals with mental health or addiction problems so that they can live with optimal health and independence.
Uschiiniichisuu Team Staff • Core clinical team: Nurse, Doctor, Community Health Representative (CHR), Social Worker, Community Worker • Mental health professionals: Psychologist/Therapist, Mental Health Nurse, Mental Health Worker, NNAADAP Worker/Addictions Worker • Allied Health Professionals : Nutritionist, Psychoeducator, Physical Therapist (PT), Occupational Therapist (OT), Speech Language Therapist (SLP) • Administrative & support personnel: Receptionist/Secretary, Administrative Technician • Case Manager (being developed)
General Health & Social Services • General medical services is provided in Current & ambulatory services • Diagnosis, treatment and contact tracing for STBBI • Basic rehabilitation assessment and intervention • Follow ups • Referral services and liaison with 2nd and 3rd line services
Psychosocial Support & Mental Health • Psychosocial support for families in difficulty • Individual therapy • Specific services for youth and families with psychosocial needs • Liaison with child and youth protection services • Referral services and liaison with 2nd and 3rd line services
Regional & Community Services • Youth Healing Services • Multi Services Day Center (MSDC) • Residential services including foster care, land-based healing and outreach activities provided at the regional level • FASD diagnostic and intervention clinic • Other local services for youth and families with special needs
Health Promotion & Prevention Services • Healthy lifestyles: physical activity, nutrition (CSB, Rec. Dept, Youth Centers, Band)* • Counselling & screening for STBBI and vaccination (CSB)* • Counselling & prevention for dental care in school (CSB)* • Prevention activities for addiction (drugs, alcohol, gambling, smoking, etc.)(CSB, Rec. Dept, CNYC, Band)*
ACCESSProject • Aim/Goals: • Improve youth engagement and awareness of mental health • Promote early identification of those in need • Provide timely access to evidence-informed, youth-friendly mental healthcare • Status summary: • Mistissini was designated as an ACCESS pilot project community. • The Resource Family Center will be the hub for this project that will be developed in partnership with the community. • Program development is underway. • Planned to start in fall 2016.
Cree Youth Clinic • Aim / Goals • Gateway for all youths aged 10 to 29 yrs to access healthcare services in their community • Annual Adolescent Clinic • Outreach Services • Status Summary • Communities consultations have been completed and summarized • Preventive Clinical Practice literature review is under way • Cree Youth Clinic Program soon the be developed
Addictions Program and Healing Lodge • Aim / Goals • To provide early intervention before severe addiction • To reinforce continuum of care by structured integrative client’s pathway in addiction services. • To have the guiding principles strongly rooted into Cree culture and values • Status Summary • Planning under way for: • EeyouIstche addiction in-patient treatment center/Healing Lodge • Community treatment services models: treatment in community • Development of a training strategy for competencies and quality services • Development of in-patient treatment program & tools
School Health Services Aim / Goals Ensure the quality of the School Health services and prevention programs offer to youth attending school in EeyouIstchee and their family. Status summary School Nurses in 6 out of 9 communities ChiiKayeh trained trainers in each community Immunization – New: Grade 4 boys & Sec3 review of chart CLE/CE: School nurses to do teaching and screening Need to promote the School Health Program to stakeholders
