HipResurfacing Patientswithadvancedarthritisofthehipmaybecandidatesforeithertraditionaltotal hipreplacement(Arthroplasty)orhipresurfacing(HipResurfacingArthroplasty).Eachof theseproceduresisatypeofhipreplacement,butthereareimportantdifferences.Your orthopaedicsurgeonwilltalkwithyouaboutthedifferentproceduresandwhich operationwouldbebestforyou. Description Thehipisaball-and-socketjoint.Inahealthyhip,thebonesarecoveredwithsmooth cartilagethatenablesthefemoralheadandacetabulumtoglidepainlesslyagainsteach other. Inatraditionaltotalhipreplacement,theheadofthethighbone(femoralhead)andthe damagedsocket(acetabulum)arebothremovedandreplacedwithmetal,plastic,or ceramiccomponents. Inhipresurfacing,thefemoralheadisnotremoved,butisinsteadtrimmedandcapped withasmoothmetalcovering.Thedamagedboneandcartilagewithinthesocketis removedandreplacedwithametalshell,justasinatraditionaltotalhipreplacement. CandidatesforSurgery Yourdoctormayrecommendsurgeryifyouhavemoreadvancedosteoarthritisand haveexhaustedthenonsurgicaltreatmentoptions.Surgeryshouldonlybeconsideredif yourhipissignificantlyaffectingthequalityofyourlifeandinterferingwithyournormal activities. Acomprehensiveevaluationbyyourorthopaedicsurgeonwillhelpyoudetermineifyou areagoodcandidateforhipresurfacing. YourSurgery BeforeSurgery Youwilllikelybeadmittedtothehospitalonthedaybeforesurgery. Beforeyourprocedure,adoctorfromtheanesthesiadepartmentwillevaluateyou.He orshewillreviewyourmedicalhistoryanddiscussanesthesiachoiceswithyou.You shouldalsohavediscussedanesthesiachoiceswithyoursurgeonduringyour preoperativeclinicvisits.Anesthesiacanbeeithergeneral(youareputtosleep)or spinal(youareawakebutyourbodyisnumbfromthewaistdown). Yoursurgeonwillalsoseeyoubeforesurgeryandsignyourhiptoverifythesurgical site. SurgicalProcedure Ahipresurfacingoperationtypicallylastsbetween11/2and3hours. Yoursurgeonwillmakeanincisioninyourthighinordertoreachthehipjoint.The femoralheadisthendislocatedoutofthesocket.Next,theheadistrimmedwith speciallydesignedpowerinstruments.Ametalcapiscementedovertheprepared femoralhead.Thecartilagethatlinesthesocketisremovedwithapowertoolcalleda reamer.Ametalcupisthenpushedintothesocketandheldinplacebyfriction betweentheboneandthemetal.Oncethecupisinplace,thefemoralheadisrelocated backintothesocketandtheincisionisclosed. Afterthesurgeryyouwillbetakentotherecoveryroom,wherenurseswillclosely monitoryouasyourecoverfromtheanesthesia.Youwillthenbetakentoyourhospital room. Youmaybeginputtingweightonyourlegimmediatelyaftersurgery,dependingonyour doctor'spreferencesandthestrengthofyourbone.Youmayneedawalker,cane,or crutchesforthefirstfewdaysorweeksuntilyoubecomecomfortableenoughtowalk withoutassistance. Aphysicaltherapistwillgiveyouexercisestohelpmaintainyourrangeofmotionand restoreyourstrength.Youwillcontinuetoseeyourorthopaedicsurgeonforfollow-up visitsinhisorherclinicatregularintervals. Youwillmostlikelyresumeyourregularactivitiesofdailylivingby6weeksafter surgery.
