Getting Started in IRIS Local Aging or Disability Resource Center: • Completes Long-Term Care Functional Screen • Verifies functional and financial eligibility • Informs about monthly IRIS allocation • Non-biased enrollment counseling • Makes referral to IRIS
Monthly IRIS Allocation • Based on information in the Functional Screen • Individualized • Predicted ongoing services cost • Geographic adjustment (urban/rural) • Excludes MA card services and infrequent expenses • Process to adjust allocations or request one-time expenses
Customized Goods and Services • Help meet needs and achieve goals related to: • living arrangement • relationships • community inclusion • work • medical or functional status • Listed on plan, allowable per Fed/State rule, • no other fund obligation, not experimental • Maintains or increases: • Safety in home or community • Skill • Community access or involvement • Maintains or decreases Medicaid service dependence
Assistance in IRIS • In IRIS, assistance is available from: • The Service Center • IRIS Consultants • Program and Planning Specialists • Fiscal Agent Services • Whoever you bring to the table!
IRIS Service Center • Service available 24hrs/day 7days/week • Solve problems, answer questions and provide information • Review and process: • New referrals • Initial plans • Plan updates • Assist with IRIS Consultant matching
IRIS Consultant Roles • Navigates, links, supports • May provide IRIS orientation, explain responsibilities and benefits of self-direction • Helps identify strengths, challenges and goals • Helps build support and service plan • Helps identify providers • Partners on developing community networks • Helps develop a back up plan
IRIS SDPC Funding IRIS Self-Directed Personal Care IRIS Allocation • People hire their own personal care workers • Hours match Medicaid Personal Care agency hours • Worker wages - $13.50/hour • IRIS Nurse visits included • Personal care can be provided outside of the home
