Colorado Mine Rescue May 2014 Problem The Destiny Mine is a small underground copper and silver operation. We employ 21 miners who work two shifts a day, five days a week. It is a multi-level, 4 level operation. Currently only one level is being mined and that is the 600 level. Mining is done with mechanized equipment. Access is via a 12 percent decline. We normally blast once each shift. The ventilation at the 600 level is supplied by a fan located at the Portal and exhausts through a borehole/secondary escapeway west of entry #1 in the main drift. The fan is on and generates 60,000 CFM . The day shift had shot off a round at the end of their shift and the afternoon shift was beginning their shift. Miner Jerry Berry was assigned to muck out the shot that had taken place one hour earlier at the face. Brad Tad, the shifter went in to check how things were going and never came out. Two other miners came out just after Brad went in and stated that they thought they had smelled rotten eggs but weren't sure and that both had bad headaches. We never had any gas problems in the past. The only communication we have underground is the one phone at the face in 4 and one in the shop. We don’t think that the one in the shop is working. Right now we have 6 miners underground and we don’t know what is going on. We do have several old mines in the area but we are not connected to any of them. The team from New Hill Mine Rescue set up a Fresh Air Base in the Main drift on the 600 level. We are a small operation with limited supplies but you just tell us what you need and we’ll try to get it. Please help us.

OR ventilate for all miners. Once vent bag miner is removed access and egress for the team will be through C 1 & 2. Before removing the barricade in D the team must line brattice and sweep the area. A Solution There is more than one way to rescue and recover the miners and the mine. We are going to assume that the team has shored up all areas in needof support, have remove any known or potentially found hazards (explosives), etc. Once the team has accessed all possible areas of the mine and before they attempt to change the ventilation they must move the FAB. The FAB should be placed in the Main drift between #1 entry and #2 entry. Stoppings: • 1. In cross cut A between entry 2 and 3. • 2. In entry 2 between cross cut A and B. • 3. In entry 3 between cross cut B and C. • 4. In cross cut C between entry 2 and 3. • 5. In cross cut C between entry 1 and 2. • 6. In the Main drift between the FAB and entry #2. • The door in Cross cut A between entry #3 and #4 can/will be closed. Some teams may choose to only place # 5 and #6 up to bring the first miner out… Some teams may try to use the 2 inch air line that is in the vent bag. The effect will be minimal and very time consuming - we should not change any placards. Keep in mind that the Muck piles located in cross cut D are impassable but are not air tight. After the vent miner is taken out the teams should tear down the stopping in Cross cut C between Entry #1 and #2. There may an attempt to take miners out through the Stopping at the FAB by traveling out in Entry #3; there will be some residual H2S in Cross cut A and Entry #2. This would cause the miners to immediately start complaining about the smell, headache and nausea… Some teams may just seal the old workings off. That will work but the time line for ventilating the mine will be substantial (2 hours). If they try to do this by using one brattice, it will not work since the shot opened up an area that is 15 feet wide.
