Introduction • This change will add functionality to capture Independent Living information in eWiSACWIS and is the first phase of several changes that will be made to meet federal NYTD (National Youth Transitional Database) requirements.
The Change • New Independent Living page – this page will be used by Independent Living coordinators (when they have access to the system) or county case workers to view and capture Independent Living information surrounding the Independent Living assessment and plan.
The Change • New Independent Living Transition Plan template – the template will be launched from the Independent Living page. • Changes to the existing Permanency Plan page and template - remove old Independent Living information.
Change • New Tickler (Independent Living Assessment and Transition Plan tickler) – the tickler will notify the worker of when they are required to create the Independent Living page. The youth become eligible for Independent Living services once they have been in an AFCARS qualifying out of home placement for a cumulative six months after the age of 15. A conversion will also be done to create ticklers for all children already in an AFCARS qualifying placement.
Page Overview • This page will be accessed via Maintain>Independent Living. The worker will need to search out the youth and hit continue. This will create a page for youth who do not have a plan in the system, or launch the pre-existing page.
Page Overview • The Independent Living page consists of three tabs: Basic, Assessment / Plan, and Services.
Page Overview • Upon accessing the Independent Living page, eWiSACWIS pre-fills the fields in the Name group box. The Youth name with associated Person ID, Youth DOB, and Gender fields contain the information of the youth selected from the Search process
Page Overview • The Options dropdown field will be displayed on every tab and contains the ‘Independent Living Transition Plan’ template.
The Basic Tab • The Basic tab is used to display demographic information as well as capture whether the youth was ever an adjudicated delinquent as well as capture documents the worker has obtained for the youth.
The Basic Tab • Participant Information • Housed in this group box will be; the Highest Grade Level Completed, Special Education Status, Race, Ethnicity, Hispanic/Latino, Indian Tribe, Indian Tribe 2, Tribal Reference #, and Was the Youth Ever Adjudicated Delinquent. All information will be prefilled from the Youth’s Person Management Record, except for the last question, which is user entered.
The Basic Tab • Documents Obtained • Housed in this group box will be; Birth Certificate, Drivers License, Education Records, Employment Information, Immigration Papers, MA Post Foster Care, Medicaid Card Information, Medical Records, School Transcripts, Selective Service, Social Security Card, State ID, and a comments section.
The Basic Tab • Documents Obtained • All information on this tab will be user entered. There will be a comments section for any added information that the user would like to document.
The Assessment/Plan Tab • The Assessment / Plan tab is used to capture dates and information surrounding the Independent Living Assessment and Independent Living Transition Plan. The text fields on the page are required by state statutes. Information entered in the text fields will prefill to the Independent Living Transition Plan template.
The Assessment/Plan Tab • Entering dates into the Independent Living Assessment Completed field and the Independent Living Transition Plan Completed field will delete the Independent Living Assessment and Transition Plan tickler.

The Assessment/Plan Tab • The first group box will be the Independent Living Assessment. Inside this box will be: the date the Independent Living Assessment was completed; the date the Independent Living Assessment was revised; and a description of the assessment process, tools and methods.
The Assessment/Plan Tab • All of this information will be user entered. The date revised need to be updated each time the Assessment is revised. Neither of the date fields are required, however, when a date is entered, a description is required.
The Assessment/Plan Tab • The second group box will be the Independent Living Transition Plan. Inside the box will be; the date the Independent Living Plan was completed, the date the Independent Living Plan was revised, and two group boxes: Anticipated age of discharge from out of home care and Anticipated living situation upon discharge from out of home care
The Assessment/Plan Tab • All of this information will be user entered. The two narrative fields will become required once an initial completion date is entered for the Plan.
The Services Tab • The Services tab displays the Independent Living services the county is providing the youth. To add services to the page, the worker will select the Insert button which opens the Independent Living Services page. Once the worker enters the information on the Independent Living Services page and saves the page, the information is summarized on the Services tab.
The Services Tab • The Service / Activity details including category, begin/end dates, Service / Activity description, responsible person and explanation are captured in the Services /Activities group box. Multiple services can be inserted on the Independent Living Services page. If a worker wishes to remove a Service / Activity, a Delete link is available next to the IL Service Category field.
The Services Tab • Once a Service / Activity has been saved, the information will be displayed on the Services tab of the Independent Living page and sorted by category. The worker can access each specific Service / Activity pop-up by clicking the Edit link.

Online Changes • Changes to existing Permanency Plan • Keep the Independent Living tab for existing permanency plans that were approved prior to the new release. All pending plans and new plans – the Independent Living tab will be removed with release 3.4.

Online Changes • Changes to existing Permanency Plan • Add a ‘Date youth became eligible for Independent Living Services’ date field. Upon Save and when launching the template. • This will be system derived based on the Independent Living tickler logic using the Perm Plan Date field as an anchor.
Online Changes • Tickler • A tickler will be created for youth in AFCARS placements that have been in care for any 6 months after the age of 15 to begin IL Services. A tickler will notify workers that they have 90 days to complete the Independent Living Assessment and the Independent Living Transition Plan.
Online Changes • Tickler • The tickler will be generated upon the creation of an AFCARS qualifying out of home placement (and hidden behind the scenes until the display dates are reached). Independent Living page is youth specific and is displayed under all the child’s cases, however only one tickler is created for the youth and is linked to the case where the youth is placed out of home.
Online Changes • Tickler • The tickler will be assigned to the Primary Worker of the case where the youth is placed out of home. • The display date will be the day the youth became eligible for Independent Living services (6 months in an AFCARS qualifying placement after the youth is the age of 15).
Online Changes • Tickler • Due date will be 90 days after the youth became eligible for Independent Living services. • Tickler will be escalated at the due 14 days prior to the due date. • NOTE: The tickler should also be created/updated upon the approval of a placement correction page.
Online Changes • Tickler • Deleted as followed: • When a placement ending is approved and the tickler has not reached the display date (6 months in qualifying placement after the age of 15). If the tickler has already reached the display date, then continue to display the tickler.
Online Changes • Tickler • Deleted as followed (continued): • When a date is entered into both the Independent Living Assessment Completed field and into the Independent Living Transition Plan Completed date field on the Independent Living page and the worker saves the page. • Note: The tickler will prevent case closure, but can be deleted by the primary worker’s supervisor.
