The “Secret Ingredient”Comprehensive Unit-Based Patient Safety Program • Form a unit CUSP team with executive sponsorship • Measure unit culture • Educate staff on Science of Safety • Identify defects using the Staff Safety Assessment; prioritize defects • Learn from one defect per quarter • Implement team/communication tools

Learn from a Defect Tool • Designed to be used by bedside staff at the unit level • Designed to rigorously define, analyze and organize the various components and conditions that contributed to a defect • Provides a logical approach to breaking down faulty system issues • Likely to be successful in the elimination of future occurrences

Timeline for Science of Safety Staff Education and Staff Safety Assessment • 4-27 to 5-11: Plan content and set up inservice schedule for Science of Safety Education and Staff Safety Assessment • 5-11 to 5-25: Conduct in-services and administer Staff Safety Assessment questionnaire • 5-25 to 6-3: Collate results of Staff Safety Assessment questionnaire

  8. Upcoming Dates • Continue planning for HSOPS survey – goal is to reach 60% response rate. • Attend ONE HSOPS Database Training Webinar (May 16, 19, 23, or 26). No need to register, just join the meeting. • Last Content Call will be May 25th– Implementation in a Community Hospital • June 1, 2011 – Data Collectionfor CAUTI rates and prevalence begins!! • June 10, 2011 – Kick Off Meeting in Columbia, Courtyard by Marriot.
