  2. ALGH/ALGCH actively seeks to find comparative data sources and elects to use national sources whenever available. If we believe that a measure is important to our success and an external comparison is not available we use the Advocate system for comparison. Our ability to obtain data is limited by increasing financial challenges as access to external data sources is costly. Health outcomes data is generally based on diagnostic and procedure codes selected from clinical documentation. Clinical documentation and coding practices may not accurately reflect true health outcomes. In addition risk adjustment methodologies vary by registry and the validity of these methodologies also is problematic. External comparisons for process data are generally not available with the exception of the core measures.

Challenges • Accurate timely data that can be managed electronically • Finding measures that are important, have useful baselines, and can be improved with cycles of improvement • Physician and associate acceptance of the data and being accountable for the outcomes • Large amount of data noise and not being able to focus on the important things

  4. Lessons Learned • Start small and focus • Explain the “Why” as part of the data analysis • Create an accountability structure for outcomes • Think out of the box
