PRE QUALIFICATIONS FOR MI SPECIFIED PERSONS • ABILITY TO READ,WRITE & EXPLAIN THE PRODUCT IN DETAIL. • GOOD TRACK RECORD OF FINANCIAL INTEGRITY. • HAVE GOOD REPUTATION IN THE SEGMENT IN WHICH HE IS WORKING AND HAVE A LONG STAY WITH NGO. • MICROINSURANCE MUST BE HIS ONE OF THE KEY ASSIGNMENT. • BELONGS TO SAME AREA IN WHICH HE IS DOING MICROINSURANCE BUSSINESS. • MOBILITY OF WORKING AREA SHOULD BE VERY FAST.

AGREEMENT FOR SPECIFIED PERSON (TERMS & CONDITION • 1 All Specified Person will have to provide Photo ID Proof (Driving License, PAN Card, Election Card) Address Proof (Electricity or Telephone Bill- last paid, Passport, Bank pass book. Two guarantors or Company may decide any alternate later on forguarantee. • All Specified Persons are required to take training from the LIC of India. • All Specified Persons will have to procure 10 policies per month to continue as Specified Person for at least five years. Thereafter 12 policies required per year to earn renewal commission till 15 years from the date of appointment and authorized to do the business in only one selected Division. • .We also caution all Specified Person that any activity of false bad propaganda against the LIC of India, reproduction and misuse of material during the prospective policy seekers with a false commitment and working against interest of company in any manner amounts to indiscipline and will attract stern action with prior notice of 7 days. • 5.Business details will have to be sent to HO up to last date of the month and commission will be paid on next month 20th day. Payout will only on minimum Rs. 300/- bill otherwise will be carried forward in the next month.

  15. 6. FP and renewal commission will be paid after receipt from LIC and with confirmation of LIC bill. (After activation by FP & Renewal Commission). 7. Any changes in the system procedure of LIC & IRDA and changes in the policy of Government which will have a direct affect on insurance business will also be invariably reflected in our agencies business. All SP/ Policyholder are bound to adhere to these changes. 8.This NGO/ Corporate Agent is not liable for any incidental or consequential damages caused by breach, cancellation, suspension of agreement, whether or not such damages are known to the NGO/ Corporate Agent, an agency appointed by the LIC of India. 9. Receipts for payment and insurance bonds of the policyholders will be sent by the LIC directly through the NGO/ Corporate Agent and the Specified Person to policyholders. NGO will also act as a co-ordinator in these activities. 10. Settlement of claims etc will be directly handled by the LIC of India only. 11. If any, fraud or misappropriation of collection is made by Specified Person. The two guarantor and team officer is responsible to compensate the said amount and said SP will be terminated as SP and will be disqualified to get the FP & Renewal commission also. 12. All legal matters can be settled under __________ legal jurisdiction only. Witness: Authorised Signatory.

BY UNDER STANDING THE NEEDS OF SP’S. • 4. MAKE UNDERSTAND THE MI SPECIFIED PERSON • ASSOCIATED WITH NGO THAT SELLING OF MI • PRODUCT IS A KIND OF SOCIAL SERVICE . • 5. BY GIVING PROPER PRODUCT KNOWLEDGE. • 6. BY SUPPLYING ALL REQUIRED FORMS , BROUCHERS ETC IN TIME.

ACTIVITIES TO BE DONE WITH NGO BEFORE STARTING THEIR BUSSINESS • MAPPING OF WORKING AREA OF SPECIFIED PERSON • AREA WISE APPOINTMENT OF TEAM LEADER • WRITE A LETTER TO SARPANCH OF EACH VILLAGES OF THEIR AREA INFORMING THAT SO & SO NGO IN YOUR AREA IS WORKING FOR SOCIAL SECURITY SCHEME OF LIC FOR THE UPLIFTMENT OF POOR. • 4. PROVIDE LIST OF VILLAGES AND SARPANCH ALONG WITH PHONE NO. IN THE SORROUNDING AREAS CAN BE OBTAINED FROM BSNL DIRECTORY , CENSUS CD.

TO ENHANCE THE PRODUCTIVITY OF SPECIFIED PERSON • EDUCATE THE SP’S FOR CONSERVATION OF BUSINESS OTHER WISE AGENCY WILL NOT SUSTAIN. • HELP THE SP’S BY THE WAY OF PUBLICITY , IN THE OPERATIONAL AREA WHERE HE WORKS FACILITATE THE MI SP’S IN THE PRESENCE OF VILLAGERS. • 8. THE MICRO INSURANCE SPECIFIED PERSONS HAVE TO BE TRAINED IN THE LOCAL LANGUAGE REGARDING THE • FUNDAMENTALS OF INSURANCE, PRODUCT FEATURES • 9.
