What is DEMENTIA? Some Causes of Dementia: • Alzheimer’s disease • Lewy’s body disease • Parkinson’s disease • Huntington’s disease • Strokes in the brain • HIV infection • Chronic drug use • Depression • Brain abnormalities Dementia is a disorder of the brain with a permanent loss of cognitive function or memory.
Pathophysiology of Dementia Primary dementias: the dementia itself is the major sign of an organic brain disease and is not directly related to any other illness, such as Alzheimer’s. Secondary dementias:those caused by some other disease - such as AIDS, head injury, stroke, multiple sclerosis, etc. Pseudo dementia:another category of dementia that is not a true dementia, but instead has similar symptoms that mimic dementia, often seen in patients with depression.
Risk Factors for Dementia • Age • Family history • Alcohol use • Atherosclerosis • Blood pressure • Cholesterol • Depression • Diabetes • Obesity • Smoking
Clinical Manifestations of Dementia • Memory loss • Difficulty with communication • Difficulty with complex tasks • Difficulty with coordination and motor functions • Getting lost • Personality changes • Inability to reason • Inappropriate behavior • Paranoia, delusions and/or hallucinations
Diagnostic Tests • CT or MRI of the brain • Evaluation by psychiatrist • Cognitive and neurological tests • Laboratory tests such as B12 deficiency or underactive thyroid
Pharmacotherapy for Dementia • Cholinesterase inhibitors: 1. Aricept 2. Exelon 3. Razadyne • Namenda • Other medications to treat symptoms, such as sleeping pills
Treatment Modalities For reversible dementia: • Take vitamins for B12 deficiency • Take thyroid medication for hypothyroid • Have surgery to remove brain tumor or relieve pressure on brain • Stop taking medications that cause confusion • Take medications to treat infection • Take antidepressants For irreversible dementia: • Help the person be independent and manage life as long as they can • Dementia medications • Counseling and family support
You Tube site evaluation My you tube video selection is from the movie “The Notebook”, which is my all time favorite movie. It is a wonderful love story with a tragic ending involving Alzheimer’s dementia. I do not think the you tube video is an accurate reflection of dementia behavior, because patients do not typically have periods of total and precise awareness. It is current and depicts a circumstance that many people are unfortunately faced with. It is told in such a manner to capture the audience’s attention while portraying the tragic nature of this disease.
