Retake Training Materials Icons Since the Retake Training Materials include only one PowerPoint presentation, these slides should be used to train school coordinators and test administrators. SC TA Fall 2006 FCAT Retake Reading and Mathematics Administration Schedule Districts have identified one of the following two weeks for their Fall 2006 FCAT Retake administration: OR Retake Training Materials Slide 4
SC TA What’s New for the Fall 2006 FCAT Retake • FCAT Administration Record—combines the Record of Absences and Invalidations and the record of attendance • FIVE elements now need to be correct on the label to make it usable: • student’s last name • student ID number • district number • school number • grade level • Because only the last four digits of the Social Security Number (XXXXX1284X) are printed on the label, school personnel must use the student PreID Roster to verify the complete number before testing begins • The document configuration has changed from a test book AND an answer document to ONE test and answer book for each test Retake Training Materials Slide 5
SC TA Reminders for the Fall 2006 FCAT Retake Forms and Procedures: • PreID Roster—lists students with a PreID label • FCAT Security Log—keeps track of the monitoring of each test room • Students using large print materials must record answers in their large print documents ONLY • Test group codes will be assigned by school coordinators Retake Training Materials Slide 6
SC TA Students to Be Tested • Grades 11–13regular and adult high school students who have not passed one or both parts of the Grade 10 FCAT • Certificate of Completion students • Three-year graduation program students (who have not passed one or both parts of the Grade 10 FCAT) in the fall of their third year in high school Retake Training Materials Slide 7
SC TA Rules for Preidentified Student Documents • Verify that the student identified on each label will be tested and that certain information on the label is correct • Use the PreID label if ALL of the following information is CORRECT (even if other information is INCORRECT) • student’s last name • student ID number • district number • school number • grade level • Grid any missing information in the appropriate sections of the grid sheet • You are NOT required to grid the missing numbers of the Social Security Number printed on the label if the PreID Roster reflects the correct Social Security Number Retake Training Materials Slide 8
SC TA Placement of Student Label Sample Student Label: Affix the label to the box in the lower leftcorner of the student grid sheet Do not remove labels once they have been affixed, and do not affix a label over another label Retake Training Materials Slide 9
SC TA Sample Student Grid Sheet Content Assessed Student Information Box This box must be completed by the student even if the document is preidentified. Test Administration Student Demographic Information Security Number This number consists of two letters followed by a six-digit number. Preidentified Area Student labels should be affixed to this area. School Use Only Box This box contains one DNS bubble and one UNDO bubble to indicate whether a test has been invalidated. SC TA Test Invalidation Policies and Procedures The DNS Bubble MUST be gridded on a USED document if: • The document is defective • Certain information on the label is incorrect • A label has been placed over another label • The student is caught cheating or is disruptive during testing The DNS Bubble MAY be gridded if: • A student becomes ill OR is not allowed the correctamount of time to finish the test Consideration: Was the student provided adequate time to respond to the test questions? • Yes—submit the document for scoring • No— invalidate the document and return it with the NOT TO BE SCORED materials Retake Training Materials Slide 11

SC TA Test Security Policies and Procedures Prohibited Activities Include: Reading the passages, test items, or performance tasks Revealing the passages, test items, or performance tasks Copying the passages, test items, or performance tasks Interpreting or reading test items or passages for students Changing or otherwise interfering with student responses to test items Causing achievement of schools to be inaccurately measured or reported Copying or reading student responses Retake Training Materials Slide 14

SC TA Security Numbers and Checklists Security Numbers: account for secure test and answerbooks consist of two letters followed by a six-digit number should be recorded each time secure test and answerbooks are distributed and returned Retake Security Checklists: list the security number of each secure book are used by the school coordinator to record the security numbers assigned to and returned by each test administrator may be copied and used by the test administrator, or security numbers may be recorded on the FCAT Administration Record Retake Training Materials Slide 16

TA Supervise Test Administration During testing, ensure that students: • clear their desks of all materials except the appropriate test materials • use only No. 2 pencils • do not have books, notes, scratch paper, or electronic devices even if they do not use them during the test • do not disturb others • work independently DO NOT: • talk with students about any test item • help students answer any test item • discuss test items or answers with students even after all test materials have been returned and testing has been completed Retake Training Materials Slide 19
TA Return Retake Materials to the School Coordinator • Verify and keep a copy of your record of security numbers (the Security Checklist, FCAT Administration Record, or a form of your own) • Remove stray reference sheets from the test materials • Check student grid sheets to ensure the following: • No stray marks in preidentified/security number area • Upper right corner was completed by each student • DNS and UNDO bubbles are accurately completed, if necessary • Complete the following: • FCAT Security Log • Online Test Administrator Comment Form • Group materials using the cover sheets in Appendix C • Return materials to the school coordinator Retake Training Materials Slide 20
SC Testing Location Considerations • Use classrooms whenever possible • Avoid testing in the cafeteria, gymnasium, auditorium, or library whenever possible • Make sure that students are not facing each other if seated at tables • Make sure that students are not in seating that allows them to easily view other students’ answers • Make sure there is sufficient space (at least three feet) between students Retake Training Materials Slide 21
SC Prepare Materials for Return to the District Coordinator • Verify return of secure materials • Complete record of security numbers (the Security Checklist, FCAT Administration Record, or a form of your own) • Make copies of and file: • Each test administrator’s record of security numbers • FCAT Security Logs • Separate test materials (by grade level and document type) • Verify accuracy of DNS and UNDO bubbles Retake Training Materials Slide 22
SC Prepare TO BE SCORED Documents for Return • Complete the following: • a Retake Document Count Form for each grade level and document type • the Answer Document Return Summary • Band stacks of documents • Grid the appropriategrade level anddocument type • Complete the school and district information • Complete the line titled Number of Documents Contained in this Paper Band • Number the paper bands: 1 of n, 2 of n, 3 of n, etc. • Place the SSS Paper Band around each stack of TO BE SCORED answer documents • Make sure that the Retake Document Count Form is on top of the banded stack labeled 1 of n Retake Training Materials Slide 23

SC TA Instructions for Completing Student Grid Sheets for Students Using Large Print Materials For a student with a PreID label: • After verifying its accuracy, affix the label to the appropriate area on the large print test and answer book For a student who does NOT have a label: • Grid the large print student grid sheet by hand Retake Training Materials Slide 32
TA Return Large Print and Braille Materials to the School Coordinator • Verify and keep a copy of your record of security numbers (the FCAT Administration Record or a form of your own) • Pull out any stray reference sheets • Verify the accuracy of each student grid sheet • Student name and district/school number appear in the appropriate locations on all test documents • DNS and UNDO bubbles are accurately completed, if necessary • Complete the online Test Administrator Comment Form Retake Training Materials Slide 33

SC Prepare Large Print and Braille Materials for Return to the District Coordinator • Verify return of secure materials and complete a record of security numbers • Make copies of and file the record of security numbers (the FCAT Administration Record or a form of your own) • Verify the following: • Each envelope contains documents for one student only • The student grid sheet is complete and accurate (check the DNS and UNDO bubbles) • Complete a Retake Document Count Form per subject tested for each student and place the completed form on top of the corresponding document(s) Retake Training Materials Slide 35
SC Prepare Large Print and Braille Materials for Return to the District Coordinator (cont.) • Make sure that each student’s envelope contains the appropriate materials for each subject tested • Place the following materials into the box(es) in which they arrived: • the completed student return envelopes • NOT TO BE SCORED secure large print and braille documents • braille scripts • braille notes • Seal the box(es) • Place a white label anywhere on the top of each box and complete the label • Return the box(es) to the district coordinator Retake Training Materials Slide 36
