Purpose, Scope, & Objectives This process establishes a procedure for affixing lockout or tagout devices to energy–isolating devices and for otherwise disabling machines or equipment to prevent unexpected energizing, startup, or release of stored energy. Its intent is to prevent injury to employees and to protect the environment. It addresses lockout/tagout practices that both Chevron, and contractors shall follow on our facilities. On Deep Water facilities, any work requiring a LO/TO, requires the initiation of a Permit To Work. This process covers the servicing and maintenance of machines and equipment in all circumstances where stored energy (electrical, mechanical, pneumatic, hydraulic, or thermal) or substances (flammable, toxic, or corrosive) may endanger personnel or property through direct or indirect exposure. In addition to this process, a separate procedure must be written for each piece of equipment that has multiple energy sources. GOM LO/TO

  3. GOM LO/TO Procedures: • Only trained, authorized personnel shall implement this lockout and tagout procedure. Before any personnel service or maintain a machine or equipment where the unexpected energizing, startup, or release of stored energy could occur and cause injury, the machine or equipment shall be isolated and rendered inoperative. Whenever replacement, repair, renovation, or modification of machines or equipment is performed, and whenever new machines or equipment are installed, energy-isolating devices for such machines or equipment shall be designed to accept a lockout device.

  6. GOM LO/TO • Tags shall meet the following requirements: • Including their means of attachment, tags shall be designed to prevent accidental removal. Means of attachment shall be non-reusable, attachable by hand, self–locking, and non-releasable, with a minimum unlocking strength of no less than 50 pounds. Tie wraps with a tensile breaking strength of 50 pounds or more meet this requirement. • Tags shall indicate the identity of the authorized person applying the devices. • Tags shall be weather–resistant and contain an intact eye so they can be secured to a piece of machinery or equipment with a tie wrap. • Only "Danger" and "Information" tags have been approved for lockout and tagout. These tags are OSHA-approved with standard coloring and format.

  7. GOM LO/TO • All tags must be dated and signed. They must also identify the equipment being tagged and the nature of the work being performed. • Equipment tagged with the "Danger" tag shall not be operated until the tag is removed by the authorized person who installed it, by the PIC or by a PIC designee. "Information" tags shall be used in addition to the "Danger" tag anytime there is insufficient room for remarks on the "Danger" tag.

Lockout devices shall be: • Standardized throughout GOMBU • Substantial enough to prevent removal without the use of excessive force • Capable of withstanding the environment to which they are exposed • Designated lockout devices shall not be used for other purposes and shall be identified as lockout devices.
