Member Responsibility Cont. • Coverage Begin & End Dates – • New Hires , Newborns, New spouse • End dates- coverage ends the date a dependent ceases to be eligible • Timely submission of “Proof of Relationship” • Timely notification of Life Status changes/ Qualifying events (31 days)

Term Life Insurance Basic & Voluntary Life • The Hartford Basic, Voluntary, Spouse and Dependent Life Insurance. • Basic Life Insurance is 140% of base annual salary up to $50,000 paid for by the City. • Term Life - No evidence of insurability required for coverage up to $250,000 • Maximum coverage amount 7 times annual salary not to exceed $500,000 • EOI required for amount over $250K
