  2. Create two Outcome Content Slides • The First Content Slide which should introduce your lesson with a sentence or two and yourself with your contact information. • The Expectation Content Slide, which presents what you were expecting to experience and accomplish as a result of participating in this project. • The Actual Learning Slide, which contains a list of actual skills and learning that has taken place (for you as the teacher, not the students) as a result of this program.

  3. Create two Outcome Content Slides • A summary of how things went for you during the program. Create a brief reflection on the SUCCESSES of your lesson implementation written to other educators who might consider implementing your lesson in their own classroom. • A summary of how things went for you during the program. Create a brief reflection on the SHORTCOMINGS of your lesson implementation written to other educators who might consider implementing your lesson in their own classroom.

  4. Create two Results Content Slide • The Student Data slide(s) include the charts and explanations you created during data analysis in Module 8. • Your 3 student work samples should also be embedded within a slide representing a Basic, Proficient and Advanced piece of work.

  5. Create one Revisions Slide • The significance of the data analysis and changes made to improve the lesson. • Also account for certain trends or patterns within your students' performance on the activities and how your modifications relate to those trends.

  6. Create one Synthesis Slide • Summarize your entire experience and how it has affected your lesson design, identification of objectives, choice of activities and use of technology to support and deliver information • * The slide should include a concluding statement about your personal/professional development as a participant of this statewide program. (We are looking for your current viewpoints on the role of standards-based lessons and technology integration to support student learning and how this program helped shape or modify your ideas in these areas.)

  7. Create one Synthesis Slide • Enhancing Your Presentation • PowerPoint makes it easy to add images, sounds, video snippets and other graphic elements to your presentation. These multimedia elements add a great deal of interest to your presentation, and you should add as many of them as practical. Here are some ideas for elements you can add to enhance your presentation: • * Images of students performing lesson activities • * Images of student work or projects • * Scans of student papers or drawings • * Video clips of students performing lesson activities • * Images or scans of your handouts or other teaching materials • * Screen grabs from Websites visited • * Scans of maps, blueprints, schematics, flow-charts

  8. Program Completion and Paperwork • When you finish creating your final presentation, you're on the home stretch for the Teaching with Technology program. You'll meet as a group to make the final presentation of your lesson plans and PowerPoint shows. • Additionally, you will need to access the CTAP2 iassessment site and re-survey in the hopes of identifying any new proficiency levels you have attained through your participation in this program.

  9. Final Program Survey of Skills • Complete the program survey at the iassessment site for those areas that you assessed your skills at the beginning of this program in module 3. • Remember, in order to complete this program, you must have all components of this program completed to the satisfaction of your mentor so that they can sign off on each of the modules and submit the required paperwork to the CTAP Region 2 Office. • * Your modified lesson should be uploaded to your lesson plan area and viewable to both your group leader (mentor) and your colleagues. • * You should have three student samples of work attached (uploaded) to your lesson that represent an example of Basic, Proficient and Advanced. • * You should have your final program presentation attached (uploaded) to your lesson plan as well as a PowerPoint presentation that contains all of the required elements identified in Module 10, including your student results graph and a summary of your program experience. • *

  10. Final Program Survey of Skills • Complete the CTAP2 iassessment survey a second time in order to capture any growth with the instructional use of technology. • * Additionally, group leaders (mentors) will be asked to identify the degree to which participants engaged in the online forums and group led discussions throughout the program modules. • Please click on the URL below to see a sample portfolio. • http://ctaponline.org/home/materials/twt_materials/mod_10/lesson_plan.html

  11. FINAL CTAP TWT PAPERWORK • Participants/Mentors Must Complete and Deliver the Following Forms • Participants must reassess their skills at the conclusion of this program using the online survey at http://ctap2.iassessment.org/. • * Download and complete the CTAP TWT Sign-Off Form with Mentor's signatures included. Print it and mail to the BCOE office. • * Download and Complete the online Payee Data Form using the following instructions as you complete the six sections of the form: • Section 1: Leave this section blank, we'll take care of this section. • Section 2: Enter your mailing address. • Section 3: Check the 'Individual/Sole Proprietor' box. • Section 4: Enter your Social Security Number. • Section 5: Check the box that describes your California residency status. • Section 6: Please sign, date and add your phone number. • Print it and mail to our office. • * In order to receive Continuing Education Units from CSU Chico, you will need to complete an additional piece of paper work: Shanna will NOW show you how to do THIS>>>>>>> • Click here to watch a short tutorial on how to fill that card out appropriately.
